Application of hyperbaric oxygen therapy in diabetic foot ulcers: A meta‐analysis

Abstract
Hyperbaric
oxygen
therapy
(HBOT)
has
been
used
in
patients
with
diabetic
foot
ulcers
(DFU)
for
many
years,
but
its
clinical
efficacy
is
still
controversial.
Therefore,
this
study
explored
the
of
HBOT
applied
to
DFU
by
means
meta‐analysis.
PubMed,
Cochrane
Library,
Embase,
CNKI
and
Wanfang
databases
were
searched,
from
database
inception
October
2023,
published
randomised
controlled
trials
(RCTs)
collected.
Two
investigators
independently
screened
collected
literature,
extracted
relevant
data
assessed
quality
literature.
Review
Manager
5.4
software
was
analysis.
Twenty‐nine
RCTs
1764
included.
According
combined
results,
when
compared
conventional
treatment,
significantly
increased
complete
healing
rate
DFUs
(46.76%
vs.
24.46%,
odds
ratio
[OR]:
2.83,
95%
CI:
2.29–3.51,
p
<
0.00001)
decreased
amputation
(26.03%
45.00%,
OR:
0.41,
0.18–0.95,
=
0.04),
incidence
adverse
events
higher
(17.37%
8.27%,
2.49,
1.35–4.57,
0.003),
whereas
there
no
significant
difference
mortality
(6.96%
12.71%,
0.52,
0.21–1.28,
0.16).
Our
results
suggest
that
effective
increasing
decreasing
DFUs,
increases
events,
while
it
effect
on
mortality.

Background
Non-healing
diabetic
foot
ulcers
(DFUs)
are
significant
risk
factors
for
amputations.
Though
the
available
literature
suggests
that
adjuvant
hyperbaric
oxygen
therapy
(HBOT)
fastens
healing
process
and
reduces
of
amputations,
its
overall
evidence
in
reduction
amputation
remains
controversial.
Thus,
present
study
aimed
to
compare
efficacy
safety
HBOT
standard
wound
care
(SWC)
with
SWC
alone
patients
DFUs.
Methods
This
prospective,
randomized,
controlled
involved
60
adult
DFU.
Based
on
simple
random
number
table,
were
equally
randomized
into
two
group:
(n=30)
(n=30).
The
received
24
sessions
(six
per
week)

period

consecutive
weeks.

Introdução:
O
pé
diabético
é
uma
complicação
da
diabetes
mellitus
que
se
não
tratada
de
forma
adequada
pode
resultar
em
amputações,
altos
custos
hospitalares,
internações
prolongadas
e
piora
qualidade
vida
das
pessoas
com
essa
patologia.
Frente
a
esta
situação,
o
presente
estudo
aborda
oxigenoterapia
hiperbárica
seu
uso
pela
enfermagem
no
tratamento
desse
tipo
lesão.
Objetivos:
evidenciar
os
efeitos
benéficos
diabético.
Metodologia:
Trata-se
revisão
integrativa
literatura,
realizada
período
fevereiro
abril
2024
portal
Biblioteca
Virtual
Saúde
(BVS),
nas
bases
dados
Medical
Literature
Analysis
and
Retrieval
System
Online
(MEDLINE),
Base
Dados
Enfermagem
(BDENF)
Literatura
Latino-americana
do
Caribe
Ciências
(LILACS),
empregando
Descritores
(DeCS):
Oxigenoterapia
AND
OR
Pé
cruzamento
dos
descritores
nos
bancos
escolhidos
para
resultou
332
resultados,
após
inserção
critérios
inclusão
exclusão,
totalizou-se
121
registros,
desses,
10
artigos
foram
selecionados
como
pertinentes
ao
estudo.
Resultados:
Os
achados
evidenciaram
profissional
enfermeiro
responsável
assistência
tanto
pré,
durante
cada
sessão
oxigenoterapia.
Quanto
aos
benefícios
dessa
terapia
diabético,
mais
importantes
foram:
acelera
cicatrização
ferida,
reduz
as
paciente,
melhora
sensação
álgica,
aumenta
sobrevida
pacientes.
Conclusão:
tem
ampliado
Brasil
um
método
adjuvante
convencional
mostrou
muito
eficaz
utilização
isolada,
sendo
protagonistas
terapia.

Beeinträchtigungen
der
Lebensqualität
und
dem
Bedarf
an
speziellen,
individualisierten
Behandlungen.
Insbesondere
im
Fall
von
Fußgeschwüren
sind
klassische
Therapien
nicht
immer
effektiv,
daher
werden
neue
Behandlungsstrategien
benötigt.
vorliegende
Übersicht
zielt
darauf
ab,
alternative
aktuellen
Wirksamkeitsdaten
für
diabetische
Fußgeschwüre
vorzustellen,
die
geeignete
Optionen
bei
refraktären
Geschwüren
sein
könnten.
Plättchenreiches
Plasma
Stammzellen
haben
eine
regenerative
entzündungshemmende
Wirkung
zeigen
gute
Ergebnisse
Behandlung
Geschwüren,
während
Ozontherapie
lokale
Durchblutung
den
Antioxidationsstatus
verbessert.
Tragbare
Geräte
könnten
entscheidend
langfristige
Überwachung
Patienten
diabetischem
Fuß
werden,
da
sie
Identifizierung
oder

frühzeitig

eingesetzt

kann.
