Abstract Aim To investigate whether the lack of keratinized mucosa (KM) affects peri‐implant health after 10 years loading. Materials and Methods Data from 74 patients with 148 implants two randomized controlled studies comparing different implant systems were included analyzed. Background: Peri-implantitis, an inflammatory condition leading to progressive bone loss around dental implants, represents a significant challenge in modern implant dentistry. Conventional mechanical debridement and adjunctive antibiotics or antiseptics often fail fully eradicate complex biofilms may promote antibiotic resistance. Photodynamic therapy (PDT) mediated by natural photosensitizers (e.g., curcumin, riboflavin, 5-aminolevulinic acid) has emerged as potential strategy for peri-implantitis management due its targeted antimicrobial anti-inflammatory effects. Objectives: This systematic review aimed evaluate the efficacy, clinical outcomes, safety of PDT treatment identify optimal protocols regarding photosensitizer concentrations, light source parameters, application techniques. Methods: Following PRISMA 2020 guidelines, databases (PubMed/Medline, Embase, Scopus, Cochrane Library) were searched from 1 January 2015 3 2025 English-language publications. Studies assessing naturally based interventions vitro diseased surfaces included. Quality assessment used Revised Risk Bias tool (RoB 2) randomized controlled trials tailored nine-item framework studies. Eleven studies met inclusion criteria.
