Childhood trauma is a key modifiable risk factor for psychopathology. Despite significant scientific advances, traumatised children still have poorer long‐term outcomes than nontraumatised children. New research paradigms are, thus, needed. To this end, the review examines three dominant assumptions about measurement, design and analytical strategies. Current warns against using prospective retrospective measures of childhood interchangeably; interpreting cross‐sectional differences in putative mediating mechanisms between adults with or without history as evidence longitudinal changes from pre‐trauma conditions; directly applying explanatory models resilience vulnerability to psychopathology forecast individual unseen cases. The warnings equally apply on broader adverse experiences (ACEs). Further examining these can generate new insights how prevent its detrimental effects.

Childhood trauma (CT) has adverse consequences on mental health across the lifespan. The understanding of how CT increases vulnerability for psychiatric disorders is growing. However, lack an integrative approach to psychological and biological mechanisms hampers further advancement. This review integrates findings explanatory levels from a longitudinal adult cohort – Netherlands Study Depression Anxiety (NESDA). We reviewed all studies (k = 37) NESDA (n 2981) published 2009 2020 containing related psychopathology potential CT. was associated with higher risk anxiety depressive strongest associations in comorbid group. predicted onset these disorders, recurrence, poorer outcomes (more comorbidity chronicity). maladaptive personality characteristics cognitions (e.g., neuroticism negative self-associations), mild stress systems dysregulations (heightened cortisol inflammation), advanced aging (increased epigenetic telomere attrition), lifestyle (higher smoking rate body mass index), somatic decline increased metabolic syndrome dysregulations), brain alterations reduced mPFC volume amygdala reactivity). Abstract Stressful experiences affect biological stress systems, such as the hypothalamic–pituitary–adrenal (HPA) axis. Life can potentially alter regulation of HPA axis and has been associated with poorer physical mental health. Little, however, is known about relative influence stressors that are encountered at different developmental periods on acute reactions in adulthood. In this study, we explored three models exposure cortisol reactivity to a modified version Trier Social Stress Test (TSST) by leveraging 37 years longitudinal data high-risk birth cohort (N = 112). The cumulative model suggests accumulated across lifespan leads dysregulated reactivity, whereas embedding implicates early childhood critical period. sensitization assumes dysregulation should only occur when high both concurrently. All predicted altered but do not anticipate its exact form. We found support for effects. However, pitted against each other, life more blunted responses age over above stress. Additional analyses revealed middle also reactivity.
