Abstract Background: Cervical cancer (CC) is largely preventable but still causes around 300,000 deaths annually, particularly among vulnerable women. Methods: This systematic review and meta-analysis included studies from four databases (MEDLINE, EMBASE, CENTRAL, ISI Web of Science) up to February 2024 (from 2004 for HIV). We examined seven groups in middle- high-income countries: women low socioeconomic status (WLSES), prisoners, sex workers, with substance use disorders (WSUD), mental illness (WMI), migrants, living HIV (WLWH). Observational on CC high-grade lesion (HGL) risk, incidence, or prevalence were included. Independent reviewers assessed all articles. The main outcome was HGL measured by incidence rate ratio (IRR), risk (RR), standardized (SIR), odds (OR). PROSPERO registration: CRD42024535331. Findings: 126 studies. Vulnerable had a significantly higher (RR 2.78, 95%CI 2.32-3.32) 2.51, 2.02-3.11), WSUD, WLWH facing 2- 5-fold increased risk. Interpretation: Marginalised face highlighting the need targeted policies improve screening treatment access. Funding: this funded CBIG-SCREEN project (The has received funding EU Horizon 2020 research innovation program under Grant Agreement No 964049).

In adjusted model, (aOR: 1.5, [95% CI 1.2-1.8, p<0.001]), than one visit centre period 4.4, [95%CI 2.8-6.8, p<0.001]) presented infectious disease 2.1, 1.5-3.1, had higher odds having disorder. Lastly, 1,840 country attended where CDSS tool implemented, 29 (1.6%) correctly identified individual. Mental is condition may be overlooked healthcare.
