Background The pursuit of sleep quality has become an important aspect people’s global quest for overall health. However, the objective neurobiological features corresponding to subjective perceptions remain poorly understood. Although previous studies have investigated relationship between electroencephalogram (EEG) and sleep, lack longitudinal follow-up raises doubts about reproducibility their findings. Objective Currently, there is a gap in research regarding stable associations EEG data assessed through multiple collection sessions, which could help identify potential targets related quality. Methods In this study, we used portable device collect resting-state prefrontal cortex over 3-month period from 42 participants (27 first month, 25 second 40 third month). Each participants’ was using Pittsburgh Sleep Quality Index (PSQI) estimate recent Results We found that significant consistent positive correlation low α band activity PSQI scores (r=0.45, P<.001). More importantly, remained across all recordings (P<.05), regardless whether considered same cohort or expanded sample size. Furthermore, discovered periodic component primarily contributed association with PSQI. Conclusions These findings represent identification reliable target sessions. Our results provide solid foundation future applications devices monitoring screening disorders broad population.

Insomnia is present in up to one third of the adult population worldwide, and it can independently or with other medical conditions such as mental, metabolic, cardiovascular diseases, which highlights importance treating this multifaceted disorder. associated an abnormal state hyperarousal (increased somatic, cognitive, cortical activation) orexin has been identified a key promotor arousal vigilance. The current standards care for treatment insomnia recommend non-pharmacological interventions (cognitive behavioural therapy) first-line and, if are not effective available, pharmacotherapy. In contrast most sleep medications used decades (benzodiazepines 'Z-drugs'), new receptor antagonists do modulate activity γ-aminobutyric acid receptors, main inhibitory mechanism central nervous system. Instead, they temporarily block pathway, causing different pattern effects, e.g., less morning next-day motor dyscoordination, cognitive impairment. pharmacokinetic/pharmacodynamic properties these drugs basis characteristics explained package inserts, including recommended starting dose. Orexin seem be devoid any dependence tolerance-inducing rendering them viable option longer-term treatment. Safety studies did show exacerbation existing respiratory problems, but more real-world safety pharmacovigilance experience needed. This review provides overview history, action, relation insomnia, features available mediating signalling.

Summary Insomnia is a stress‐related sleep disorder conceptualised within diathesis‐stress framework, which it thought to result from predisposing factors interacting with precipitating stressful events that trigger the development of insomnia. Among genetics and epigenetics may play role. A systematic review current evidence for genetic epigenetic basis insomnia was conducted according Preferred Reporting Items Systematic Reviews Meta‐Analyses (PRISMA) system. total 24 studies were collected twins family heritability, 55 genome‐wide association studies, 26 about candidate genes insomnia, eight epigenetics. Data showed complex polygenic disorder, likely be caused by synergy environmental factors, reactivity being important trait. Summary Insomnia disorder is considered as a stress‐related associated with hyperarousal, stress and emotion dysregulation the instability of ‘flip‐flop’ switch system. The orexinergic system well known for its key role in sleep arousal processes but also allostatic regulating emotions may thus be major interest insomnia treatment. Accordingly, we discuss potential orexins on processes, brain systems modulating implications pathophysiology. We reviewed available data effect dual orexin receptor antagonists (DORAs) stress/emotions present our findings narrative review. Few animals humans have reported that disrupted related to overactivation system, while some more consistent response acute disorders. Taken together these let us hypothesise an hyperarousal hyperactivation arousal‐promoting insomnia. On other hand, it possible by rebalancing DORAs regulate both systems, turn, contributing ‘switch off’ Nevertheless, studies are needed clarify evaluate effects sleep, systems.
