Insomnia and obstructive sleep apnoea are the two most common disorders frequently co-exist. Patients with comorbid insomnia experience worse daytime function, mental health physical than patients either disorder alone. General practitioners may face unique challenges in assessment management of this prevalent debilitating condition. This article aims to provide an overview prevalence, consequences, Australian general practice.Patients or should be assessed for both conditions. Treatments offered The recommended treatment is cognitive behavioural therapy, whereas first-line moderate severe lifestyle/weight advice (where relevant) continuous positive airway pressure therapy.

Biological, environmental, behavioral, and social factors can influence sleep lead to disorders or diseases. Sleep are common, numerous heterogeneous in terms of their etiology, pathogenesis, symptomatology. Management sleep-wake circadian (SWCD) includes education hygiene, behavioral strategies, psychotherapy (cognitive therapy (CBT), particularly), instrument-based treatments (i.e. positive airway pressure therapy, hypoglossal nerve stimulation), pharmacotherapy. Depending on the disease, varies is executed sequentially, be a combination several forms therapy. Drugs used for SWCD include traditional wake-promoting agents, chronotherapeutic agents. Recently, novel medications, which more precisely acting specific neurochemical systems orexin system) important wake, also increasingly being used. In this review, pharmacotherapy common (insomnia, sleep-related breathing disorder, central hypersomnolence, rhythm wake disorders, parasomnias, movement disorders) embedded overall therapeutic concept each disorder presented. There an outlook possible future pharmacotherapies.

Introduction Clinical presentation of both insomnia and obstructive sleep apnea (COMISA) is common. Approximately 30% clinical cohorts with OSA have symptoms vice versa. The underlying pathophysiology COMISA multifactorial. This poses a complex challenge. Currently, there are no guidelines or recommendations outside continuous positive airway pressure (CPAP) therapy cognitive behavioral for (CBTi). Clinically translatable precision medicine approaches to characterize individual causes endotypes may help optimize future pharmacological management COMISA.

Insomnia treatment among individuals with comorbid insomnia and obstructive sleep apnea is suboptimal. In a pilot randomized controlled trial, 19 were allocated to one of two arms: EX + EX, consisting 8-week phases exercise training (EX), or RE CBTiEX, encompassing 8 weeks relaxation (RE) followed by combined cognitive-behavioral therapy (CBTiEX). Outcomes included Severity Index (ISI), polysomnography, cardiorespiratory fitness measures. A mixed-model analysis variance revealed Group × Time interaction on peak oxygen consumption change, F (1, 14) = 10.1, p .007, increased ( .03, g ′ −0.41) reduced ISI .001, 0.82) compared .49, 0.16) post-8 weeks.
