Light contains different wavelengths of the spectrum that are detected and received by photoreceptor cells (rod cone cells) in retina. However, light serves non-visual functions human perception, such as emotion regulation, cognitive performance, sleep cycle, hormone production. These alter brain activity oxygen consumption brain. Our objective was to investigate effect on using NIRS. Ten subjects (8 males, 2 females) with normal color vision were exposed achromatic (white gray) or chromatic (blue, green, yellow, red) for 20 seconds random order, followed 30 rest (dim light). This stimulation-rest cycle repeated 4 times (total time = 20.30 minutes). NIRS used measure Oxy-Hb, Deoxy-Hb, Total-Hb levels frontal lobe during stimulation. After stimulation, participants assessed BRUMS a effectiveness test. The results indicated z-score Oxy-Hb significantly altered response gray compared white light. Red blue led greater than green yellow there no significant Deoxy-Hb both exposure. Gray induced feelings depression confusion, while red affected tension anger. promoted vigor. Fatigue did not show difference between Additionally, long had more positive short wavelengths. findings indicate possible uses neurotherapeutic interventions stimulating individuals impairments therapy boxes, well architectural lighting design impact regulation.

Summary Progress in the field of insomnia since 2017 necessitated this update European Insomnia Guideline. Recommendations for diagnostic procedure and its comorbidities are: clinical interview (encompassing sleep medical history); use questionnaires diaries (and physical examination additional measures where indicated) (A). Actigraphy is not recommended routine evaluation (C), but may be useful differential‐diagnostic purposes Polysomnography should used to evaluate other disorders if suspected (i.e. periodic limb movement disorder, sleep‐related breathing disorders, etc.), treatment‐resistant (A) indications (B). Cognitive‐behavioural therapy as first‐line treatment chronic adults any age (including patients with comorbidities), either applied in‐person or digitally When cognitive‐behavioural sufficiently effective, a pharmacological intervention can offered Benzodiazepines (A), benzodiazepine receptor agonists daridorexant low‐dose sedating antidepressants (B) short‐term (≤ 4 weeks). Longer‐term these substances initiated some cases, considering advantages disadvantages Orexin antagonists periods up 3 months longer cases Prolonged‐release melatonin ≥ 55 years Antihistaminergic drugs, antipsychotics, fast‐release melatonin, ramelteon phytotherapeutics are Light exercise interventions adjunct therapies

Sleep disorders constitute a significant disruption for shift workers. Beyond medical interventions, phototherapy is recognized as an effective approach to significantly alleviate sleep disorders, particularly among individuals engaged in work. However, the dose and efficacy evaluation of have not yet been determined. This study conducted systematic review across five databases from January 1, 1990, December 31, 2023. A total 11 articles were selected meta-analysis using random-effects model. The results showed that light therapy improved time (TST) (MD = 32.54, p < 0.00001) efficiency (SE) 2.91, 0.007) workers compared control group. Subgroup analysis regression implied medium illuminance (900–6000 lx) long treatment duration (≥ 1 h) during night was more extending time, whereas higher-illuminance increasing (lx*h) beneficial SE. In summary, has degree overall workers, findings current contribute reference evidence setting experimental design on workers' clinical research.

Insomnia disorder, characterized by a complaint of reduced sleep quality or quantity and associated daytime impairment, is highly prevalent with life productivity. Cognitive behavioural therapy for insomnia (CBT-I) the current first-line treatment chronic disorder. Here, we outline our perspective future optimization psychotherapeutic insomnia. We identified following areas as most promising: first, optimizing efficacy CBT-I protocol; second, developing diagnostic therapeutic approaches non-responders partial responders; third, advancing widespread implementation psychotherapy More specifically, how protocol could be optimized through an improved understanding mechanisms, discuss potential adaptive strategies. Another promising approach improving using add-ons such physical exercise circadian-based interventions. Both may in certain subgroups patients In terms non-response, identify acceptance commitment (ACT-I) to CBT-I. ACT-I, however, still needs evaluated actual Implementing clinical practice one major challenges at hand. brief treatment, targeted challenging patient groups, digital help improve implementation. For research agenda, suggest that further into randomized-controlled trials CBT-I, focus on science have bring field forward.

Background: Insomnia is increasingly recognized for its marked impact on public health and often associated with various adverse outcomes, including cardiovascular diseases mental disorders. The aim of this study was to investigate the efficacy pre-sleep dim light therapy (LT) as a non-pharmacological intervention insomnia in adults, assessing influence sleep parameters circadian rhythms. Methods: A randomized, open-label, two-arm clinical trial conducted over two weeks 40 participants aged 20–60 years, all whom had disorders (CRIS, KCT0008501). They were allocated into control LT groups. group received exposure warm-colored light, minimizing blue spectrum, before bedtime. combined subjective evaluation via validated, sleep-related questionnaires, objective assessments actigraphy, molecular analyses clock gene expression peripheral blood mononuclear cells. Baseline characteristics between groups compared using an independent t-test continuous variables chi-squared test categorical variables. Within-group differences assessed paired t-test. Changes analyzed linear regression, adjusting each baseline value body mass index. patterns changes calculated mixed model. Results: exhibited significant improvements quality (difference difference [95% CI]; −2.00 [−3.58, −0.43], efficiency (LT: 84.98 vs. control: 82.11, p = 0.032), advanced Dim Light Melatonin Onset (approximately 30 min). Molecular analysis indicated reduction CRY1 after LT, suggesting signals regulation. Conclusions: This provides evidence improving rhythm alignment adults insomnia. Despite limitations, such small sample size short duration, results underscore potential viable approach Future research should expand these larger more diverse cohorts followed longer period validate further elucidate medicine.
