Background The increasing prevalence of fibrosis in donor livers raises concerns about its impact on post-transplantation outcomes, though this relationship remains unclear. This study aims to assess the effect liver patient and graft survival following transplantation. Methods Data from UNOS-SRTR registry (1987–2024) were analyzed, focusing patients who received transplants with biopsy-proven fibrosis. cohort was stratified based grade, outcomes compared using Cox regression Kaplan–Meier analysis. Competing risk models applied specific causes for failure, subgroup analyses explored sensitivity transplant outcomes. Results Of 22,897 patients, 17,926 non-fibrotic grafts, 4,971 had grafts varying grades. Donor associated age, steatosis, portal infiltrate, generally affecting those better overall condition. Significant differences observed ( p = 0.001) 0.002) between non-fibrosis groups. Further analysis revealed that increased malignancy 0.028), cardiovascular disease 0.017), respiratory failure 0.033), but showed lower rejection rates at six months one year. Background/Objectives: Metabolic dysfunction-associated steatotic liver disease (MASLD) is a growing global health problem and the discovery of drugs challenging. In this study, we aimed to investigate effects intestinal activation X receptor (LXR)α on MASH. Methods: An intestinal-specific LXRα model in mice was established subjected MASH development by combining Western diet carbon tetrachloride. Lipid metabolism, reverse cholesterol transport (RCT), steatosis, inflammation, fibrosis were evaluated. vitro models steatosis used explore role scavenger class B type 1 (SRB1). Results: We found that improved several MASLD features, including levels triglycerides, RCT, systemic hepatic inflammatory profiles, fibrosis. These associated with increased high-density lipoprotein (HDL) SRB1 expression. depletion hampered beneficial HDL fibrogenesis cells altering both peroxisome proliferator-activated receptors γ small mothers against decapentaplegic homolog protein (SMAD)2/3 proteins. Conclusions: Our findings showed parallel induction are protective advanced MASLD.
