ABSTRACT Inflammatory bowel diseases are chronic inflammatory conditions with growing prevalence in western populations. Iron is an essential component of erythrocytes hemoglobin. Under the influence elevated hepcidin production, iron sequestered cells during inflammation which, turn, leads to restriction for red blood synthesis. As a consequence, deficiency and anemia most prevalent extraintestinal complications IBD patients. Patients thus treated oral supplements that have limited efficacy as absorption blunted intestinal inflammation. Moreover, supplementation can cause previous studies shown worsens response. However, comparative analysis effects low, adequate high dietary content matching given patients has not been performed mice. We therefore tested impact deprivation murine model colitis induced by dextran sodium sulfate. found both were accompanied more severe earlier signs gastrointestinal bleeding compared mice fed diet. Highlights•Drugs currently used against irAEs reduce anti-tumor immunity•ECP induces adiponectin production in inflamed organs while sparing tumor tissue•ECP high response rates patients with ICI-induced irAEsSummaryImmune-related adverse events (irAEs) cancer receiving immune checkpoint inhibitors (ICIs) cause morbidity and necessitate cessation of treatment. Comparing irAE treatments, we find that immunity is preserved mice after extracorporeal photopheresis (ECP) but reduced glucocorticosteroids, TNFα blockade, α4β7-integrin inhibition. Local elicits a tissue-specific effect by reducing pro-inflammatory T cell frequencies the colon tumor-specific development. A prospective phase-1b/2 trial (EudraCT-No.2021-002073-26) 14 reveals low ECP-related toxicity. Overall rate for all 92% (95% confidence interval [CI]: 63.97%–99.81%); colitis-specific complete remission 100% CI: 63.06%–100%). Glucocorticosteroid dosages could be ECP therapy. The ECP-adiponectin axis reduces intestinal tissue-resident memory activation CD4+IFN-γ+ cells colitis without evidence loss immunity. In conclusion, identify as an immunomodulatory molecule controls blocking immunity.Graphical abstract

Abstract Hematopoietic stem cell transplantation (HSCT) is a highly effective therapy for malignant blood illnesses that pose high risk, as well diseases are at risk due to other variables, such genetics. However, the prevalence of graft-versus-host disease (GVHD) has impeded its widespread use. Ensuring stability microbial varieties and associated metabolites crucial supporting metabolic processes, preventing pathogen intrusion, modulating immune system. Consequently, it significantly affects overall well-being susceptibility host disease. Patients undergoing allogeneic hematopoietic (allo-HSCT) may experience disruption in balance between system gut bacteria when treated with medicines foreign cells. This can lead secondary intestinal inflammation GVHD. Thus, GM both reliable indicator post-transplant mortality means enhancing GVHD prevention treatment after allo-HSCT. be achieved through various strategies, including nutritional support, probiotics, selective use antibiotics, fecal microbiota (FMT) target microbes. review examines research advancements practical following These findings offer novel insights into

Abstract In the randomized, double-blind, phase 3 GRAPHITE study (NCT03657160), anti-α 4 β 7 integrin antibody vedolizumab showed greater efficacy than placebo for prevention of lower-gastrointestinal (GI) acute graft-versus-host disease (aGVHD) after unrelated allogenic hematopoietic stem cell transplantation (allo-HSCT). This post hoc analysis assessed and safety versus lower-GI aGVHD in Japanese non-Japanese patients, when added to standard GVHD prophylaxis (calcineurin inhibitor + methotrexate/mycophenolate mofetil / − anti-thymocyte globulin [ATG]). Steroid-refractory acute graft-versus-host disease (SR-AGVHD) presents a significant barrier to successful outcomes following allogeneic hematopoietic cell transplantation (HCT), despite advancements in GVHD prophylaxis and management. While ruxolitinib therapy has shown improved response rates, survival benefits remain elusive. This review explores the definitions proposed distinct pathophysiology treatment landscape of SR-AGVHD. Emerging therapies offer potential, yet further research is critical better define steroid-refractory populations, improve precision with biomarkers, overcome resistance, particularly ruxolitinib-refractory cases.
