SummaryBackgroundCarbapenem-resistant hypervirulent Klebsiella pneumoniae (CR-hvKp) has been increasingly reported worldwide, posing a severe challenge to public health; however, the mechanisms driving its emergence and global dissemination remain unclear.MethodsWe analysed CR-hvKp strains derived from canonical hvKp backgrounds, acquired carbapenemase-encoding gene. These were identified 485 CRKp isolates in CRACKLE-2 China cohort, 259 multi-centre study, 67,631 K. genomes available GenBank. Clinical harbouring IncFIIK34 KPC-2 plasmid selected for genome sequencing, RNA-Seq, conjugation assays, vivo, ex vitro phenotypic characterisation.FindingsAnalysis of clinical 414 24 countries GenBank an as prevalent KPC (detected 25%, 45/178 KPC-producing CR-hvKp). Despite a scarcity of data, before 2022 Ukraine was already considered high-prevalence country for carbapenemase-producing Enterobacterales (CPE), and the situation has dramatically worsened during full-scale war with Russia. The aim this study to analyse CPEs isolated in Poland from victims Ukraine. included 65 CPE isolates March till February 2023, recovered 36 Polish medical centres 57 patients arriving Ukraine, differing largely by age reason hospitalisation. All were sequenced MiSeq ten Klebsiella pneumoniae also MinION. Taxonomy, clonality resistomes analysed all CPEs, whereas phylogeny, serotypes, virulomes plasmids characterised K. pneumoniae, partially Escherichia coli ST131, using various bioinformatic tools. Multifactorial diversity reflected patients' clinical-epidemiological heterogeneity. represented six species. most prevalent 50 15 sequence types (STs), mainly ST395, ST307, ST11, ST147 ST23, producing NDM (-1/-5), OXA-48 (-48/-1242) or KPC (-2/-3)-like carbapenemases. Each STs produced groups loosely related isolates, clusters close relatives and/or unique correlating K serotypes Many these, especially NDM-1- OXA-48-producing ST395 Russian organisms. Others, example, NDM-1-producing clustered those Poland. Numerous had specific virulence genes, including aerobactin iuc, due spread pNDM-MAR plasmids, showing both resistance virulence. Two E. Carbapenem-resistant and virulence plasmid-harboring Klebsiella pneumoniae (pVir-CRKP) has emerged spread globally, yet clinical investigations from the United States remain limited. We conducted a genomic analysis of 884 unique carbapenem-resistant K. isolates multicenter US cohort identified 6 pVir-CRKP isolates, including 2 sequence type (ST) 23, ST893, ST11 isolates. Patients infected with experienced high Pitt bacteremia scores 33% 30-day mortality rate. The exhibited significant variation in genes plasmids, along differences mucoviscosity, capsule production, survival normal human serum, resistance to killing by polymorphonuclear neutrophils, vivo pathogenicity. Phylogenetic analyses showed that most were genetically similar strains reported other global regions. emergence higher potential carbapenem than predominant clone underscores need for active surveillance.

Hypervirulent Klebsiella pneumoniae (hv Kp ) has emerged as a pathogen of global concern associated with invasive community-acquired infections. The combination hypervirulence and carbapenem resistance can result in severe difficult-to-treat This retrospective study aimed to investigate the spread hv sequence type 23 (ST23) Ireland convergence hypervirulent (hv) antimicrobial genotypes. Short-read sequences (PE300) for 90 K. ST23 isolates were generated by Galway Reference Laboratory Services (GRLS).
