Abstract Background In recent years, projects to develop reporting guidelines have attempted integrate the perspectives of patients and public members. Best practices for patient involvement (PPI) in such not yet been established. We recently developed an extension PRISMA (Preferred Reporting Items Systematic Reviews Meta-Analyses), be used systematic reviews outcome measurement instruments (OMIs): PRISMA-COSMIN (COnsensus-based Standards selection health Measurement INstruments) OMIs 2024. Patients members formed a small but impactful stakeholder group. critically evaluated PPI component this project recommendations conducting when developing guidelines. Main text A partner was integral research team member at development grant application stage. Once started, five contributors (PPCs) were recruited participate Delphi study; three PPCs contributed subsequent steps. collected quantitative feedback through surveys; qualitative garnered focus group discussion after study debrief meetings activities. Feedback thematically combined with reflections from team, predominantly positive. The following themes emerged: importance partnership, number involved, onboarding, design surveys, flexibility process, complexity methodological research, power imbalances. Impacts on content presentation guideline evident, reciprocal learning between occurred throughout project. Lessons learned translated into 17 future projects.
