Abstract Background Aboriginal and Torres Strait Islander people experience intergenerational trauma as a legacy of the impacts colonisation. Replanting Birthing Trees (RBT) aims to transform compounding cycles harm positively reinforcing nurturing recovery for parents babies. This paper describes protocol developmental evaluation culturally responsive, trauma-aware, healing-informed, continuity care(r) model support during first 2000 days (pregnancy, birth five years after birth). Methods The RBT project will be conducted in partnership with seven health services across Victoria (Royal Women’s Hospital Mercy Women) Western Australia (WA) (Armadale Hospital, Australian Country Health Service (Northam, Narrogin, Moora Merredin)), Australia. consists workstreams: resource repository including framework; validated sensitive enquiry tools; workforce development training; toolkit; strategies families stay together from start. Consolidated Framework Implementation Research (CFIR) informs implementation strategies. Acceptability, feasibility, costs effectiveness evaluated using mixed methods analysis qualitative quantitative data, collected key stakeholder interviews; parent service provider discussion groups cost audit; knowledge attitude practice surveys; pre post outcome data; interrupted time series routinely administrative linked co-design workshops. Competitive funding human research ethics committee approval were assessed against Indigenous excellence criteria protocols ensure cultural emotional safety participants communities. Discussion Participatory action approaches are used foster reflective on data within process. Findings shared newsletters, plain language summaries, presentations publications.
