Objective This study analyzed the status and factors associated with having a steady homosexual partner (HP) among men who have sex (MSM) to provide scientific basis for developing prevention control strategies acquired immunodeficiency syndrome in this population. Background: Pre‐exposure prophylaxis (PrEP) usage has slowly increased in the United States, but significant disparities persist across race, ethnicity, sex, gender, age, and geography. Determinants of PrEP inequities include stigma medical mistrust, lack patient-centered services, access to clinical care, organizational resistance change—within a health care system that neglects these barriers. Methods: We describe 5 implementation strategies providing nontraditional settings underserved populations, using an equity-based approach address key structural determinants. The alternative used Ending HIV Epidemic projects (community-based organizations, telePrEP, mobile clinics, pharmacies, emergency departments) were chosen for setting characteristics their serving structurally populations . Results: Community-based organizations have earned trust within communities can serve as hubs comprehensive sexual including PrEP. Telehealth, which expanded significantly because COVID-19, help overcome transportation scheduling barriers access. Mobile clinics also broaden delivery by bringing tailored services directly communities, often shorter wait times extended hours. HIV incidence increased by 18% between 2015 and 2019 among American Indians (AIs) despite declining rates in other racial/ethnic groups. Culturally-appropriate implementation of prevention programs is needed to address the intersectional conditions contributing vulnerabilities experienced AIs.
