Abstract Objectives To outline the design of P3 study and serve as a summary interim baseline patient characteristics. Methods is multicenter, prospective cohort targeting 1000 acute ischemic stroke (AIS) cerebral small vessel disease (CSVD) patients with 2‐year follow‐up from 80 participating hospitals across China. Comprehensive multimodal imaging, neuropsychological tests, biological samples were collected prospectively on admission visits. Patients interviewed face to for 2 years followed up. Results Until 30 December 2023, 642 enrolled 67 centers. In AIS cohort, 219 (72.5%) diagnosed post‐stroke cognitive impairment (PSCI). Compared those without PSCI, PSCI group exhibited significantly lower levels education history (all P < 0.05). CSVD 53 (41.4%) impairment. normal function, impaired function had higher prevalence hypertension diabetes All completed 20 well clinical information blood sample collection. Nearly 95% underwent structural MRI ASL, 60% fMRI DKI or DTI. Conclusions aims establish comprehensive spatiotemporal profile VCI. Through multidimensional analysis information, radiomics, proteomics, metabolomics, microbiomics, genetics, provide more understanding VCI trajectories individual variability, enhancing early detection prognosis management.

Worldwide, around 50 million people live with dementia, and this number is projected to triple by 2050. It has been estimated that 20% of all dementia cases have a predominant cerebrovascular pathology, while perhaps another vascular diseases contribute mixed picture. Therefore, the contribution affects 20 currently will increase markedly in next few decades, particularly lower- middle-income countries. In review, we discuss mechanisms cognitive impairment (VCI) review management. VCI refers spectrum pathologies any degree impairment, ranging from subjective decline, mild dementia. While acute decline occurring soon after stroke most recognized form VCI, chronic disease, particular cerebral small-vessel can cause insidious absence stroke. Moreover, disease not only commonly co-occurs Alzheimer’s (AD) increases probability AD pathology result clinical but may also etiologically development pathologies. Despite its enormous health economic impact, neglected research area, adequately powered trials therapies, resulting proven treatments. Current management emphasizes prevention treatment risk factors, evidence for intensive hypertension control. Reperfusion therapies attenuate VCI. Associated behavioral symptoms such as apathy poststroke emotionalism are common. We highlight novel strategies hopefully lead new course-modifying therapies. Finally, importance including endpoints large cardiovascular need an increased focus funding important area.

Introduction: The risk of dementia in patients with stroke-heart syndrome (SHS) remains unexplored. Patients and methods: Retrospective analysis using the TriNetX network, including ischaemic stroke from 2010 to 2020. These were categorised into two groups: those SHS (heart failure, myocardial infarction, ventricular fibrillation, or Takotsubo cardiomyopathy within 30 days post-stroke) without SHS. primary outcome was 1-year (vascular dementia, other disease, unspecified Alzheimer’s disease). secondary all-cause death. Cox regression after 1:1 propensity score matching (PSM) performed calculate hazard ratios (HRs) 95% confidence intervals (CIs) for outcomes. Results: We included 52,971 (66.6 ± 14.6 years, 42.2% females) 854,232 (64.7 15.4 48.2% females). Following PSM, 52,970 well-balanced considered each group. had a higher incident compared (HR 1.28, 95%CI 1.20–1.36). highest during first 31 follow-up 1.51, 1.31–1.74) mainly driven by vascular mixed forms. increased SHS, independent oral anticoagulant use, sex age but it aged <75 years ⩾75 years. Discussion conclusion: is associated dementia. Future studies are needed develop innovative strategies preventing complications improving long-term prognosis these patients.
