Bronchiectasis has an increasing prevalence and substantial clinical economic burden. Therefore, physicians should identify patients with bronchiectasis at high risk of disease progression to ensure optimal management in advance. The heterogeneity means it is unlikely that any single parameter could highrisk patients; therefore, severity usually assessed using validated composite tools, such as the Severity Index, FACED, Aetiology Comorbidity predict long-term outcomes bronchiectasis. Disease severity, however, implies advanced process lung destruction. Earlier intervention may prevent improve outcomes. To risk, rather than established disease, we need shift our focus from activity. activity denotes activation level underlying pathophysiological processes can be measured presentations biomarkers. This review discusses a paradigm management, focusing on progression.
