Diabetic kidney disease (DKD), one of the most severe complications diabetes mellitus (DM), has received considerable attention owing to its increasing prevalence and contribution chronic (CKD) end-stage (ESRD). However, use drugs targeting DKD remains limited. Recent data suggest that long non-coding RNAs (lncRNAs) play a vital role in development DKD. <b><i>Introduction:</i></b> Focal segmental glomerulosclerosis (FSGS) is a common glomerulopathy with an unclear mechanism. The demand for FSGS clinical diagnostic biomarkers has not yet been met. Circular RNA (circRNA) novel non-coding multiple functions, but its value remains unexplored. Although diabetic kidney disease (DKD) is responsible for more than half of all chronic and end-stage (ESKD), the association light (LM) electron microscopic (EM) structural changes with clinical parameters prognosis in DKD incompletely understood.This an interim analysis 62 patients diagnosed biopsy-confirmed from multicenter TRIDENT (Transformative Research Diabetic Nephropathy) study. Twelve LM 8 EM descriptors, representing glomeruli, tubulointerstitium, vasculature were analyzed their relationship measures renal function. Patients followed every 6 months.Multivariable linear regression revealed that estimated glomerular filtration rate (eGFR) upon enrollment correlated best interstitial fibrosis. On other hand, function decline (eGFR slope) most lesions including global glomerulosclerosis mesangiolysis. Unbiased clustering based on histopathologic data identified 3 subgroups. The first cluster, encompassing subjects mildest histologic lesions, had preserved second third clusters similar degrees dysfunction damage, but differed degree epithelial cell podocyte injury (podocytopathy subtype). Cox proportional hazard showed cluster 2 highest risk to reach ESKD (hazard ratio: 17.89; 95% confidence interval: 2.13-149.79).
