Introduction Advances have been made in understanding the aetiology of functional neurological disorder (FND); however, its pathophysiological mechanisms not definitively demonstrated. Evidence suggests interacting roles for altered emotional processing and interoception, elevated autonomic arousal, dissociation, but there is limited evidence demonstrating their causal influence on specific FND symptoms. Our superordinate aim to elucidate potentially shared distinct aetiological factors two common subtypes, seizures (FS) motor symptoms (FMS). Methods This study has a multimodal, mixed between- within-groups design. The target sample 50 individuals with FS, FMS, clinical controls (anxiety/depression), healthy controls. Potential (e.g., adverse life events, physical/mental health symptoms, dissociative tendencies, interoceptive insight/sensibility) will be assessed detailed medical history interview self-report questionnaires. A laboratory session include neurocognitive battery, psychophysiological testing, cardiac interoception time estimation tasks an isometric handgrip task. subsample undergo magnetic resonance imaging, including structural, resting-state task-based scans combined recording. Remote monitoring ecological momentary assessment wearables measure variability potential predictors/correlates ≥2 weeks patients’ daily lives. Longitudinal follow-ups at 3, 6, 12-months monitor longer-term outcomes groups. Discussion employs multimodal research methods rigorously examine several putative FND, subjective/experiential, behavioural, physiological levels. test hypotheses about role processing, dissociation initiation or exacerbation directly comparing these processes FS FMS first kind, reveal important targets prevention treatment future.

Remote monitoring technologies (RMT) could provide critical insights into the mechanisms underlying functional neurological symptoms (FNS). We examined feasibility and acceptability of a novel RMT protocol, aiming to identify psychobiological correlates antecedents FNS in everyday life. Seventeen individuals with (seizures/motor) 17 healthy controls (HC) completed ecological momentary assessments (EMA) eight times daily for 1-week, reporting severity, associated physical psychological symptoms, subjectively significant events. Sleep quality was reported daily. Fitbit Charge 5s measured objective physiological variables. Multilevel modelling variables variability. EMA completion rates were high (≥80%). At week-level, group significantly elevated subjective arousal, pain, fatigue, dissociation, negative affect, events, stressful sleep duration, compared HC. Objective disturbance resting heartrate, also greater sample. severity correlated fatigue disturbance, at day- or within-day levels. Daily events affect temporally predicted severity. RMTs are feasible acceptable tools investigation real-world settings, revealing as possible triggers FNS. Larger-scale, longer-term studies needed FND.
