Antibiotic resistance patterns of the major human periodontal pathogen Porphyromonas gingivalis were assessed over a 20-year period in United States. Subgingival P. was cultured pre-treatment from 2193 severe periodontitis patients during three time periods: 1999-2000 (936 patients), 2009-2010 (685 and 2019-2020 (572 patients). The clinical isolates tested for vitro to 4 mg/L clindamycin doxycycline, 8 amoxicillin, 16 metronidazole, with post hoc combination data metronidazole plus amoxicillin. Clindamycin-resistant significantly more prevalent (9.1% patients) (9.3%; 15-fold increase) as compared (0.6%). amoxicillin also increased 0.1% 1.3% 2.8% (28-fold 2019-2020. doxycycline low (≤0.5% prevalence), statistically unchanged, period. This mapping review highlights the need for a new paradigm in understanding of peri-implantitis pathogenesis. The biofilm-mediated inflammation and bone dysregulation (BIND) hypothesis is proposed, focusing on relationship between biofilm, inflammation, biology. close interactions immune cells are discussed, with multiple stable states likely existing clinically observable definitions peri-implant health peri-implantitis. framework presented aims to explain transition from disease as staged incremental process, where factors contribute distinct steps towards tipping point manifested clinically. Peri-implantitis and periodontitis are both polymicrobial diseases induced by subgingival plaque accumulation, with some differing clinical features. Studies on the microbial gene transcription activity of peri-implantitis microbiota limited. This study aimed to verify hypothesis that disease-specific lead features, using an integrated metagenomic, metatranscriptomic, network analysis. Metagenomic data in were obtained from same 21 subjects metatranscriptomic 12 a database. The co-occurrence based metagenomic analysis had more diverse species taxa correlations than Solobacterium moorei Prevotella denticola high core specific network. Moreover, plasmin receptor/glyceraldehyde-3-phosphate dehydrogenase genes was higher peri-implantitis. The implementation of adjunctive antibiotics has been recommended for the therapy peri-implantitis (PI). In this review, antibiotic resistance patterns in PI patients were assessed. A systematic scoping review observational studies and trials was established conjunction with PRISMA extension reviews. SCOPUS, PubMed/MEDLINE, EMBASE, SCIELO, Web Science, LILACS databases reviewed along gray literature. primary electronic examination produced 139 investigations. Finally, four met selection criteria. These evaluated 214 implants 168 patients. Porphyromonas gingivalis Fusobacterium nucleatum mainly presented high to tetracycline, metronidazole, erythromycin Similarly, Aggregatibacter actinomycetemcomitans also highly resistant clindamycin doxycycline. Other microorganisms such as Tannerella forsythia, Parvimonas micra, Prevotella intermedia/nigrescens significant levels other including amoxicillin, azithromycin, moxifloxacin. However, most did not show combination amoxicillin metronidazole. Although management antimicrobials is controversial, relevant used treat PI, usually prescribed dentistry, observed. Clinicians should consider demonstrated treatment its public health consequences.
