Endobronchialultrasound (EBUS)-guidedintralesional chemotherapy and thermal ablation using cryotherapy electrocautery can be used for the management of malignant central airway obstruction (CAO). This is a case 62-year-old male presenting with cough, hoarseness, shortness breath. He was diagnosed squamous cell carcinoma stage IVA (T4N2M1a), causing CAO that progressed to impending respiratory failure (ECOG PS 4). Chest CT scan findings included left main bronchus mass measuring 3.7 x 5.8 5.9 cm complete atelectasis, right pleural-based mass, osteolytic destruction 3rd lateral ribs. Bronchoscopy showed fungating, friable in carina extending orifice mainstem 70% 100% occlusion, respectively. A multimodality treatment approach taken tumor debulking by electrocautery, EBUS-guided intralesional cisplatin, systemic chemotherapy. Subsequently, there an interval decrease size multiple areas, lung re-expansion. The clinical symptoms patient significantly improved ECOG status increased 1.
