Introduction First-mile walking (FMW) supports sustainable mobility and public health by enabling utilitarian recreational walking. However, its relationship with neighbourhood walkability physical activity remains underexplored in tropical, high-density cities like Bangkok. This study examines these associations. Methods A cross-sectional survey of 881 Bangkok residents from 50 districts assessed (NEWS-A) (GPAQ). Multivariable logistic regression models examined associations between walkability, FMW, sufficient (≥150 min/week). Results Higher significantly increased both FMW. High-rise were more likely to engage walking, while planned high-rise neighbourhoods supported Utilitarian strongly correlated achieving activity. Conclusion Neighbourhood design plays a critical role promoting Policy efforts should prioritise pedestrian infrastructure, green spaces, transport connectivity encourage active lifestyles.

The global ageing population is increasing. As their physical functions deteriorate, older adults face not only health challenges but also mental issues.
