Diabetes mellitus (DM) poses a significant challenge to global health, with its prevalence projected rise dramatically by 2045. This narrative review explores the bidirectional relationship between periodontitis (PD) and type 1 diabetes (T1DM), focusing on cellular molecular mechanisms derived from interplay oral microbiota host immune response. A comprehensive search of studies published 2008 2023 was conducted elucidate association these two diseases. Preclinical clinical evidence suggests relationship, individuals T1DM exhibiting heightened susceptibility periodontitis, vice versa. The includes recent findings human studies, revealing variations in composition patients, including increases certain pathogenic species such as

Diabetes mellitus is a main risk factor for periodontitis, but until now, the underlying molecular mechanisms remain unclear. can increase pathogenicity of periodontal microbiota and inflammatory/host immune response periodontium. Hyperglycemia induces reactive oxygen species (ROS) production enhances oxidative stress (OS), exacerbating tissue destruction. Furthermore, alveolar bone resorption damage epigenetic changes in induced by diabetes may also contribute to periodontitis. We will review latest clinical data on evidence promoting susceptibility periodontitis from epidemiological, mechanistic, potential therapeutic targets discuss possible mechanistic targets, focusing particular novel OS. Understanding intertwined pathogenesis explain cross-interference between endocrine metabolic inflammatory diseases better, provide theoretical basis new systemic holistic treatment, promote interprofessional collaboration physicians dentists.

Abstract Periodontitis is a chronic inflammatory disease caused by the local microbiome and host immune response, resulting in periodontal structure damage even tooth loss. Scaling root planning combined with antibiotics are conventional means of nonsurgical treatment periodontitis, but they insufficient to fully heal periodontitis due intractable bacterial attachment drug resistance. Novel effective therapeutic options clinical therapy remain scarce. Nanotherapeutics achieve stable cell targeting, oral retention smart release great flexibility changing chemical composition or physical characteristics nanoparticles. Meanwhile, protectiveness high surface area volume ratio nanoparticles enable loading, ensuring remarkable efficacy. Currently, combination advanced novel strategies most active research treatment. In this review, we first introduce pathogenesis then summarize state-of-the-art nanotherapeutic based on triple concerto antibacterial activity, immunomodulation periodontium regeneration, particularly focusing mechanism ingenious design nanomedicines. Finally, challenges prospects nano for discussed from perspective current problems future development trends. Graphical

Porphyromonas gingivalis is a gram-negative, anaerobic bacterium that lives in the oral cavity. It an integral part of microbiome, which includes more than 500 types bacteria. Under certain circumstances, as consequence virulence factors, it can become very destructive and proliferate to many cells periodontal lesions. one causative agents present extremely often dental plaque main etiological factor development disease. During various therapeutic procedures, P. enter blood disseminate through distant organs. This primarily refers influence on subacute endocarditis facilitate coronary heart disease, atherosclerosis, ischemic infarction. The action facilitated by numerous factors pathogenicity such fimbriae, hemolysin, hemagglutinin, capsules, outer membrane vesicles, lipopolysaccharides, gingipains. A special problem possibility biofilm formation. 1000 times less sensitive antimicrobial drugs planktonic cells, represents significant treatment infections caused this pathogen.

Periodontitis and diabetes are two major global health problems despite their prevalence being significantly underreported underestimated. Both epidemiological intervention studies show a bidirectional relationship between periodontitis diabetes. The hypothesis of potential causal link the diseases is corroborated by recent in experimental animals that identified mechanisms whereby can adversely affect each other. Herein, we will review clinical data on existence two-way discuss possible mechanistic interactions both directions, focusing particular new highlighting importance host response. Moreover, address trained immunity may represent unifying mechanism explaining intertwined association periodontitis. Achieving better insight clustering infectious, inflammatory, metabolic provide therapeutic options to reduce risk diabetes-associated comorbidities.

The mode and mechanism of diabetic periodontitis-induced alveolar-osteocyte death are still unclear. This study aimed to investigate the occurrence ferroptosis in alveolar osteocytes during periodontitis therapeutic potential resveratrol alleviate osteocyte ferroptosis. Diabetic was induced C57/BL6-male mice treated with or without resveratrol. Periodontitis pathogenicity analyzed by micro-CT histology, immunohistochemistry. MLOY4 were P. gingivalis-derived lipopolysaccharide (LPS)+advanced glycosylated end products (AGEs) mimicking condition vitro, ferrostatin-1 (ferroptosis inhibitor). Osteocyte expression inflammatory mediators analyzed. aggravated inhibited GPX4 SLC7A11 alleviated these effects. LPS+AGEs triggered vitro as indicated downregulated SLC7A11, upregulated malondialdehyde, disrupted mitochondrial morphology, overexpressed pro-inflammatory IL-1β, TNF-α, SOST, RANKL, IL-6, treatment reversed pIKBα pNF-κB p65 osteocytes, this effect. In conclusion, triggers possibly via disruption SLC7A11/GPX4 axis, has correct biological event.

Introduction: Periodontitis is a chronic inflammatory oral disease that destroys soft and hard periodontal support tissues. Multiple cell death modes including apoptosis, necroptosis, pyroptosis, ferroptosis play crucial role in the pathogenicity of diseases. This study aimed to identify genes associated with ferroptosis, pyroptosis different cells present periodontium periodontitis patients. Methods: Gingival tissues' mRNA sequencing dataset GSE173078 12 healthy control patients' microarray GSE10334 63 controls 64 were obtained from Gene Expression Omnibus (GEO) database. A total 910 differentially expressed (DEGs) intersected ferroptosis-related obtain differential (DCDEGs), expression levels 21 verified GSE10334. Results: Bioinformatic analysis revealed attributed necroptosis patients compared controls. Ontology (GO) Kyoto Encyclopedia Genes Genomes (KEGG) pathway enrichment analyses related various cellular immunological pathways responses, osteoclast differentiation. Additionally, single-cell RNA (scRNA) data GSE171213 4 5 tissue was analyzed clustering types death. We found among DCDEGs, SLC2A3, FPR2, TREM1, IL1B mainly upregulated neutrophils overlapping IL-1B TREM1 FPR2 are SLC2A3 ferroptosis. Finally, we utilized CIBERSORT algorithm assess association between DCDEGs immune infiltration phenotypes, based on gene profile The results positively correlated neutrophil periodontium. Discussion: findings provide as future research direction mode mechanism their pathogenicity.
