Background Noninvasive mechanical ventilation (NIV) is recommended as the initial mode of to treat acute respiratory failure in patients with AECOPD. The Ventilation Outcomes (NIVO) score has been proposed evaluate prognosis AECOPD requiring assisted NIV. However, it not validated Chinese patients. Methods We used data from MAGNET Registry study, which a prospective, noninterventional, multicenter, real-world study conducted between September 2017 and July 2021 China. Data for potential risk factors mortality were collected NIVO was calculated, in-hospital evaluated using score. Results A total 1164 included 57 (4.9%) died during their hospital stay. Multiple logistic regression analysis revealed that age ≥75 years, DBP <60 mmHg, Glasgow Coma Scale ≤14, anemia BUN >7 mmol/L independent predictors mortality. associated an increase level difference statistically significant ( p < .001). showed acceptable accuracy predicting NIV (AUC: 0.657, 95% CI: 0.584-0.729, Conclusion Our findings identified receiving NIV, providing useful information identify severe guide management predictive value patients, additional studies are needed develop validate scores based on specific populations.

Chronic obstructive pulmonary disease (COPD) accounts for 545 million people living with chronic respiratory disorders and is the third leading cause of morbidity mortality around world. COPD a progressive disease, characterized by episodes acute worsening symptoms such as cough, dyspnea, sputum production.

Eosinophils were common inflammatory cells involved in the occurrence and development of various diseases. Multiple recent studies have pointed to increasingly important role eosinophils respiratory This article aims compare expression differences blood eosinophil counts between asthma, chronic obstructive pulmonary disease (COPD), asthma-COPD overlap (ACO).Patients with COPD, ACO who seen First Affiliated Hospital Guangzhou Medical University from January 2012 June 2019 included. We collected information such as age, gender, diagnosis, medical records. Moreover, levels 10 cytokines plasma each group detected by using Meso Scale Discovery method. We included 9787 patients 15806 831 patients. From our results, it can be first found that age-related three diseases (asthma ACO: p < 0.001; COPD: P = 0.001); asthma number males was more significant than females (asthma: 0.012). Second, had higher those COPD (p 0.001). we out highly expressed stable all Finally, most showed a downward trend when level low, whereas results reversed patients; 7 similar trends patients. In conclusion, their own unique endotypes patients, which reflected fluctuation changes cytokine secretion.

The relationship between increased blood eosinophils (EOS) and the prognosis of patients with chronic obstructive pulmonary disease (COPD) remains controversial. We aimed to explore stability in multiple hospitalizations for acute exacerbations (AECOPD) over a 1-year period its readmission rates mortality.Prospectively include at least 2 AECOPD 1 year June 2019 December 2021. Using 150 cells/ul as cut-off value, study population was divided into EOS, non-EOS, fluctuating groups based on longitudinal EOS. EOS rate mortality analyzed according 6-month follow-up after hospital discharge.A total 202 were included. 48, 108, 46 groups, respectively. 77.2%. risk death lower group compared non-EOS (HR=0.323, 95% CI 0.113-0.930, P =0.036). both (HR=0.486, 0.256-0.923, =0.027) (HR=0.575, 0.347-0.954, = 0.032) than group. The COPD is relatively stable year. Patients consistently high had all-cause discharge; higher readmission.

Purpose: Patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) exhibit heterogeneous responses to corticosteroid treatment. We aimed determine whether combining eosinophil levels other routine clinical indicators can enhance the predictability treatment outcomes and come up a scoring system.Patients Methods: Consecutive patients admitted AECOPD receiving between July 2013 March 2022 at Beijing Chao-Yang Hospital were retrospectively analyzed.Data on patients' demographics, smoking status, hospitalization for in previous year, comorbidities, blood laboratory tests, in-hospital collected.Least absolute shrinkage selection operator (LASSO) regression backward logistic used predictor selection, predictive nomograms developed. The discrimination calibration assessed using area under receiver operating curve (AUC) plots.Internal validation was performed 500bootstrap method, utility evaluated decision analysis (DCA).Results: Among 3254 included, 804 (24.7%) had failure.A nomogram eosinophils, platelets, C-reactive protein (CRP), low density lipoprotein cholesterol, prognostic nutritional index (PNI), ischemic heart diseases hepatic developed predict failure history. For without history, CRP, PNI, developed.Although AUCs these two only 0.644 0.647 respectively, they significantly superior predictions based solely levels. Conclusion:We easy-to-use comprehensive utilizing readily available biomarkers related inflammation, nutrition immunity, offering modestly enhanced value corticosteroid-treated AECOPD.Further investigations into novel additional patient data are imperative optimize performance.
