ABSTRACT Background Increasing shortfalls in nursing workforces are detrimental to safety critical patient care. In general practice England up one-in-two posts predicted be unfilled by 2030/31, with Wales similarly threatened. This is reflected internationally. Limited attention has been paid how cultural and structural issues affect retention of nurses practice. The aim our study understand factors that challenge support stay Methods We conducted an exploratory qualitative interview n=41 members teams working in, or who have worked as well nurse leaders associated across Wales. Recruitment was through professional social media networks snowballing techniques. Data were collected between October 2023-June 2024 analysed following framework analysis. University York ethics approval (Ref: HSRGC/2023/586/A) gained. funded the General Nursing Council Trust. Results Recognition value central at all levels affected several themes: essence practice; commodification deprofessionalisation Opportunities for development; Employment outside National Health Service; Lack voice, precarity position lack recourse; Tipping points. Conclusion Cultural impacted on While some supported retention, others revealed deep-seated, complex which require addressing practice, local national organisational levels. Nurses experience leave them vulnerable underserved. Policy makers, employers organisations need work enable not only survive, but thrive. Cancer treatment services face considerable pressures across the SACT pathway. The availability of a subcutaneous formulation Tecentriq (atezolizumab) presents opportunities to decentralise delivery new venues. With sufficient workforce training and flexibility, decentralisation could produce lasting improvement in outcomes. Six case studies are presented highlighting successful examples service using (SC) atezolizumab, covering their impacts on patients.
