ABSTRACT Pediatric respiratory diseases, ranging from acute infections to chronic lung conditions, pose significant morbidity and mortality risks. Current treatment approaches focus on symptomatic relief rather than regenerative solutions. Stem cell therapy has emerged as a promising alternative due its potential for immunomodulation, inflammation reduction, tissue repair. Mesenchymal stem cells (MSCs) induced pluripotent (iPSCs) have been extensively studied their properties, with MSC‐derived extracellular vesicles showing cell‐free therapeutic approach. Despite preclinical early clinical findings, challenges such immune rejection, ethical concerns, the need standardized protocols hinder translation. This review examines recent advancements in pediatric outlining potential, limitations, future research directions.
