Abstract For centuries, the search for nutritional interventions to underpin cardiovascular treatment and prevention guidelines has contributed rapid development of field dietary patterns cardiometabolic disease (CMD). Numerous studies have demonstrated that healthy with emphasis on food‐based recommendations are gold standard extending lifespan reducing risks CMD mortality. Abstract Aims To quantify the relationship between self-reported, long-term lifestyle changes (smoking, waist circumference, physical activity, and alcohol consumption) clinical outcomes in patients with established cardiovascular disease (CVD). Methods results Data were used from 2011 participants (78% male, age 57 ± 9 years) Utrecht Cardiovascular Cohort—Second Manifestations of ARTerial cohort who returned for a re-assessment visit (SMART2) after ∼10 years. Self-reported change was classified as persistently healthy, improved, worsened, or unhealthy. Cox proportional hazard models to risk (cardiovascular) mortality incident Type 2 diabetes (T2D). Cardiometabolic diseases (CMDs), the leading causes of death in low-income and middle-income countries (LMICs), are proven to be mitigated through structured lifestyle interventions (SLIs-dietary changes, physical activity, tobacco cessation alcohol intake), but challenge lies scaling them up LMICs. On the contrary, regular exercise known from antiquity to be beneficial (CV) effects decreased mortality.The benefits have been confirmed by many studies, but trajectories different modes PA are not well recognized.
