Background/Objectives: Pancreatic ductal adenocarcinoma (PDAC) is a highly aggressive malignancy with poor early detection rates owing to the limited sensitivity and specificity of current biomarker CA19-9. Gut microbiota dysbiosis plays key role in PDAC pathogenesis. This study aimed evaluate noninvasive approach we developed, combining metagenome-derived microbial signatures CA19-9, improve detection. Methods: included 50 treatment-naïve patients their matched controls. Fecal samples were analyzed using shotgun metagenomic sequencing. Machine learning algorithms used develop validate diagnostic panel integrating CA19-9 levels. Subgroup analyses confirm robustness markers. Results: The combined models at both species genus levels effectively distinguished from healthy individuals, strong efficacy accuracy demonstrated through rigorous validation. Conclusions: In conclusion, combination gut microbiome profiling improves compared use alone, showing promise for diagnosis.
