Introduction Formulating and implementing mental health policy is foundational to public health. The implementation of varies in low- middle-income countries (LMICs), with some having better outcomes than others. Low poses challenges relating addressing the high burden wide treatment gaps conditions. While different strategies are applied implement contexts, there little knowledge what category being used how why their produces varied outcomes. To this end, we propose conduct a realist synthesis explain how, why, for whom, under system conditions certain work or not LMICs. Methods analysis This paper will detail protocol on conducting review literature Realist reviews/syntheses theory-driven reviews designed formulate test (confirm, refute, refine) initial programme theories whom contexts programme, intervention works as intended not. Theory built by exploring abstracting context-mechanism-outcome (CMO) configurations data. These CMO identified through retroductive theorizing, mechanism-centred approach theory development. We adopt these steps guide process synthesis: i) identify research question, clarifying scope formulating (ii) background searches PubMed, PsycINFO, Africa-Wide Information, African. Index Medicus (AIM), CINAHL Scopus databases, grey (iii) appraising quality studies data extraction (iv) synthesising Registration part evaluation LMICs registered PROSPERO (registration number: CRD42024580312). Las legislaciones en salud mental presentan una serie de problemas carácter histórico que afectan el diseño e implementación las mismas, especialmente naciones vías desarrollo, tanto que, los países más desarrollados, políticas se diseñan y ejecutan con base estudios longitudinales ensayos clínicos diferentes poblaciones tenga cada nación. El objetivo este artículo revisión ha consistido hacer minuciosa inspección bases datos científicas académicas, fin obtener un cuerpo teórico lo suficientemente sólido como para entender dificultades técnicas atraviesan muchas economías emergentes materia mental, pues no llevan a cabo, evaluaciones estandarizadas sus muestras es precisamente ahí donde radica la mayor falla ministerios programas estos países. Se concluye estudio al afirmar si desarrollo optan manera obligatoria por crear propios instrumentos evaluación desde perspectiva devenida epidemiología, psicología experimental neuropsicología, podrá ser posible apliquen, sean objetivas rigurosas postulan neurociencias cognitivas potente herramienta guiar esa tarea construcción asesoría diplomáticos políticos.

Abstract Epidemiological research over the past two decades has highlighted substance use disparities that affect Native Hawaiian and Pacific Islander youth, lack of effective approaches to address such (Okamoto et al. in Asian American Journal Psychology 10(3):239–248, 2019). The Ho‘ouna Pono curriculum is a culturally grounded, teacher-implemented, video-enhanced prevention program demonstrated efficacy rural Hawaiʻi large-scale trial Despite its potential ameliorate health youth use, programs as have been poorly disseminated implemented across Hawaiʻi, raising question: Why are not used communities most need them? present study concept mapping understand previously identified implementation barriers develop strategies for Pono. Seven Department Education (HIDOE) educational leaders administrators sorted (e.g., “There HIDOE funding support curricula”), named concepts, rated barriers’ perceived impact difficulty. Multidimensional scaling cluster analysis yielded five-cluster solution: (1) Kumu (Hawaiian word teacher) Controlled, (2) School Level Buy-in, (3) Curriculum, (4) Student Attitudes + Mindsets (Family Community), (5) Policy. Participant ratings eight high-impact low-difficulty barriers. Discussion revealed important intersections among indicating coordinated cross-level sustainment. Brainstormed using participants’ own language participatory methods school settings bidirectionally share ways best sustain programs.
