Background Patients with lymph node(LN)metastasis-positive Lung adenocarcinoma(LUAD)suffer from a significantly reduced five-year survival rate. Abstract Background The increasing incidence of early-stage T1 gastric cancer (GC) underscores the need for accurate preoperative risk stratification lymph node metastasis (LNM). Current pathological assessments often misclassify patients, leading to unnecessary radical surgeries. Methods Through analysis transcriptomic data from public databases and GC tissues, we identified a 4-mRNA panel (SDS, TESMIN, NEB, GRB14). We developed validated Risk Stratification Assessment (RSA) model combining this with clinical features using surgical specimens (training cohort: n = 218; validation 186), gastroscopic biopsies ( 122), liquid 147; 168). Results RSA demonstrated excellent predictive accuracy LNM in AUC 0.890, 0.878), (AUC 0.928), 0.873, 0.852). This study proposes a novel non-invasive diagnostic approach utilizing transcriptomic profiling of liquid biopsy samples for the early detection occult peritoneal metastases in locally advanced gastric cancer (LAGC). Postoperative recurrence remains a major challenge in patients with locally advanced gastric cancer (LAGC). Identifying reliable biomarkers for predicting can guide clinical decision-making and improve patient outcomes. This study aimed to investigate the association between four peripheral blood metabolic markers postoperative LAGC patients, develop predictive model based on these markers. retrospective cohort analyzed data from 1,040 who underwent radical surgical resection January 2010 December 2019. Peripheral indicators, including low-density lipoprotein/high-density lipoprotein (LHR), cholesterol/high-density (TCHR), triglycerides/high-density (TGHR), triglycerides × fasting glucose (TyG), were used assess status.
