Conventional hematology/oncology fellowship training is designed to foster careers in academic practice through intensive exposure clinical and laboratory research. Even so, a notable proportion of graduating fellows opt pursue clinically focused career outside the realm medicine. Given corresponding shortage oncologists nonurban rural settings, improving representation hematologists/oncologists community setting national priority. We reviewed current challenges changing models cancer care delivery context traditional model along with trends patterns for recent graduates. We defined Academic-Community hybrid (ACH) how it supports evolution contemporary care. then drew on authors' experiences formulate an innovative goal-concordant paradigm seeking ACH model. The pathway emphasizes optimizes professional development domains including care, patient safety quality improvement, business operations, equity access, healthy policy alignment organizations, medical education. This novel provides optimizing preparedness increasingly complex environment while addressing workforce shortages health disparities.

Implementation science focuses on enhancing the widespread uptake of evidence-based interventions into routine practice to improve population health. However, optimizing implementation promote health equity in domestic and global resource-limited settings requires considering historical sociopolitical processes (e.g., colonization, structural racism) centering local sociocultural indigenous cultures values. This review weaves together principles decolonization antiracism inform critical reflexive perspectives partnerships that incorporate a focus science, with goal making progress toward equity. Objective: African Americans (AAs) carry the largest burden for almost every type of cancer in US and are also more likely to die from cancer. Approximately 10% cancers can be explained by a hereditary factor detected earlier. Many AAs, however, have inequitable access risk assessment (HCRA) tools information, further exacerbating disparities rates. Innovative communication strategies promote community-based HCRA information promise as means encouraging optimal primary screening among AAs. The current pilot study followed participatory process where researchers engaged with Community Advisory Board (CAB) explore how Communication Asset Mapping (CAM) could assist lay health advisors dissemination evidence-based HC/RA within AA faith communities. Methods: research team CAB conducted exploratory community-engaged group discussions residents (n = 21) guided Infrastructure Theory, used mapping inform development CAM strategy. Results: Through textual analysis, following conclusions were reached: (1) locations (e.g., community centers) specified neighborhood networks should representatives who trusted ambassadors dissemination; (2) member voices fully represent network community-engagement process; (3) well-known frequented geographic provide true representation participants' neighborhoods create robust concerning HCRA. Conclusions: appreciated engagement they felt that its impact was limited due lack their identify important resources dissemination.
