ABSTRACT Background To evaluate the clinical presentation, pathological features and outcomes of retinoblastoma based on race origin in a global cohort patients. Methods Retrospective collaborative study 1426 patients who underwent primary enucleation for retinoblastoma. Results Patients were grouped into Caucasians ( n = 231, 16%), Asians 841, 59%), Hispanics 226, Arabs 96, 7%) Others (Africans, African Americans, Indigenous Australians; 32, 2%) cohorts. On histopathology, massive choroidal invasion was higher (30%) (26%) than (15%, p < 0.001). Post‐laminar optic nerve (28%), (20%) (9%) (11%, At mean follow‐up 41 months (median, 35 months; range, 1–149 months), tumour recurrence metastasis‐related death (9% 12%, respectively), (4% 13%, respectively) (6% 6%, respectively). Multivariate Cox proportional hazards analysis with 8th edition AJCC pT stage adjuvant therapy as covariates revealed 6.8 times greater risk orbital compared to 0.010) 3.2 0.028 0.038, Conclusion The histopathological primarily enucleated eyes vary race. Despite adjusting staging treatment, remains an independent predictor outcomes, including death. A stringent more aggressive treatment approach is recommended manifest high‐risk features.
