Abstract Background Maintaining functional status, defined as the ability to perform daily activities such bathing, dressing, and preparing meals, is central older adults’ quality of life, health, remain independent. Methods We will conduct hybrid type 2 implementation-effectiveness cluster-randomized adaptive trial at 8 VHA sites using Practical, Robust Implementation Sustainability Model (PRISM) guide implementation evaluation. During Pre-Implementation phase, we engage clinical partners develop local adaptations maximize intervention-setting fit. an launch standard bundle strategies (coalition building, champions, technical assistance) system-level audit feedback, identify with low uptake, randomize those receive continued vs. enhanced (standard plus clinician-level feedback). outcome reach (proportion eligible patients each site who screening/assessment) effectiveness appropriate identified related referrals). Discussion Implementing potential veterans. Improved includes increasing access services supports family caregivers, reducing potentially preventable acute utilization, allowing live least restrictive setting long possible. also data inform delivery proactive interventions prevent delay development independence.
