Radiation exposure causes cancer and non-cancer health effects, each of which differs greatly in the shape dose–response curve, latency, persistency, recurrence, curability, fatality impact on quality life. In recent decades, for dose limitation purposes, International Commission Radiological Protection has divided such diverse effects into tissue reactions (formerly termed non-stochastic deterministic effects) stochastic effects. On one hand, effective limits aim to reduce risks (cancer/heritable are based detriment-adjusted nominal risk coefficients, assuming a linear-non-threshold response rate effectiveness factor 2. other equivalent avoid (vision-impairing cataracts cosmetically unacceptable skin changes) threshold dose. Compared to conventional photon-based external beam radiation (PhXRT), carbon ion radiotherapy (CIRT) has superior dose distribution, higher linear energy transfer (LET), and a relative biological effectiveness (RBE). This enhanced RBE is driven by unique DNA damage signature characterized clustered lesions that overwhelm the repair capacity of malignant cells. These physical radiobiological characteristics imbue heavy ions with potent tumoricidal capacity, while having potential for simultaneously maximally sparing normal tissues. Thus, CIRT could potentially be used treat some most difficult tumors, including those are hypoxic, radio-resistant, or deep-seated. Clinical data, mostly from Japan Germany, promising, favorable oncologic outcomes acceptable toxicity. In this manuscript, we review rationales CIRT, an emphasis on repair, as well providing comprehensive overview translational clinical data using CIRT.

In June 1994, the world's first clinical center offering carbon ion radiotherapy opened at National Institute of Radiological Science (NIRS), Japan. Among several types species, ions were chosen for cancer therapy because they judged to have most optimal properties in terms superior physical and biological characteristics. As March 2010, 5,196 patients been registered radiotherapy. Clinical results shown that has potential provide a sufficient radiation dose tumor, while having acceptable morbidity surrounding normal tissues. Tumors appear respond favorably include locally advanced tumors as well histologically non-squamous cell tumor such adenocarcinoma, adenoid cystic carcinoma, malignant melanoma, hepatoma, bone/soft tissue sarcoma. By taking advantage unique ions, treatment with small fractions within short period successfully carried out variety tumors. This means can offer larger numbers than is possible other modalities over same time period.

Increasing use of heavy ions for cancer therapy and concerns from exposure to charged particles in space necessitate the study basic biological mechanisms associated with ions. As most critical damage induced by ionizing radiation is DNA double strand break (DSB), this review focuses on DSBs their repair processes. Compared X- or gamma-rays, high-linear energy transfer (LET) ion induces more complex damage, categorized into non-DSB oxidative clustered lesions (OCDL). This complexity makes process difficult, partially due retarded enzymatic activities, leading increased chromosome aberrations cell death. In general, following LET-dependent, but nonhomologous end joining defective cells, trend less emphasized.
