Sensorineural hearing-loss (SHL) is accompanied by changes in the entire ear-brain pathway and its connected regions. While hearing-aid (HA) partially compensates for SHL, speech perception abilities often continue to remain poor, resulting consequences everyday activities. Repetitive-Transcranial-Magnetic-Stimulation (rTMS) promotes cortical network plasticity may enhance language comprehension SHL patients. 26 patients using HA with were randomly assigned a treatment protocol consisting of five consecutive days either real (Active group: 13 patients) or placebo rTMS (Sham 14 patients). The stimulation parameters as follows: 2-second trains at 10 Hz, 4-second inter-train-interval, 1800 pulses. Neuronavigated was applied over left superior temporal sulcus. Audiological tests administered before (T0), immediately after (T1), one week following completion (T2) evaluate reception threshold (SRT) Pure Tone Average (PTA). In context general improvement likely due learning, active induced significant reduction SRT PTA T1 T2 versus Sham. long-lasting effects on observed Active group indicates that auditory cortex could promote sustained neuromodulatory-induced brain, improving complex sentences pure tones skills. Five enhances overall intelligibility

Abstract Speech and music are two fundamental modes of human communication. Lateralisation key processes underlying their perception has been related both to the distinct sensitivity low-level spectrotemporal acoustic features top-down attention. However, interplay between bottom-up needs be clarified. In present study, we investigated contribution acoustics attention melodies or sentences lateralisation in fMRI functional network topology. We used sung speech stimuli selectively filtered temporal spectral modulation domains with crossed balanced verbal melodic content. Perception decreased degradation information, whereas degradation. Applying graph theoretical metrics on connectivity matrices, found that local clustering, reflecting specialisation, linearly increased when cues crucial for task goal were incrementally degraded. These effects occurred a bilateral fronto-temporo-parietal processing temporally degraded right auditory regions spectrally melodies. contrast, global topology remained stable across conditions. findings suggest partially depends an goals under attentional demands.
