Abstract Objectives This article describes the challenges faced by National Library of Medicine with rise artificial intelligence (AI) and access to human knowledge through large language models (LLMs). Background Significance The AI as a tool for acceleration falsification science is impacting every aspect transformation data information, knowledge, wisdom scientific processes. Approach perspective discusses philosophical foundations, threats, opportunities revolution proposal restructuring mission (NLM), part Institutes Health, central role guardian integrity in an era AI-driven science. Results NLM can new posed working from its foundations theories Information Science embracing roles. Three paths are proposed: (1) Become Authentication Authority For Data, Information, Knowledge Systems Scientific Provenance; (2) An Observatory State Human Health supporting living systematic reviews; (3) A hub Culturally Appropriate Bespoke Translation, Transformation, Summarization different users (patients, public, well scientists clinicians) using technologies. Discussion Adapting Internet developing worldwide-web-accessible resources allowed heights. Bold moves needed adapt but offer similar prospects more significant impacts on advancement health.
