The cornea, as a dynamic and responsive tissue, constantly interacts with mechanical forces in order to maintain its structural integrity, barrier function, transparency refractive power. Descemet’s Stripping Only (DSO) is a surgical technique that utilizes the peripheral corneal endothelial cell (CEnC) migration for wound closure. Ripasudil, Rho-associated protein kinase (ROCK) inhibitor, has shown potential in DSO treatment; however, its mechanism promoting CEnC remains unclear. We observed ripasudil-treated immortalized normal and Fuchs dystrophy (FECD) cells exhibited significantly enhanced healing, particularly effective FECD cells. Ripasudil upregulated mRNA expression of Snail Family Transcriptional Repressor (SNAI1/2) Vimentin (VIM) while decreasing Cadherin (CDH1), indicating endothelial-to-mesenchymal transition (EMT) activation. activated Rac1, driving actin-related complex (ARPC2) to leading edge, facilitating migration. Ex vivo studies on cadaveric membrane (DM) showed increased proliferation CEnCs after ripasudil treatment. An ex model demonstrated from DM stroma with ripasudil. Abstract Fuchs endothelial corneal dystrophy (FECD) is a complex disease characterized by the progressive decline and morphological changes of cells (CECs) that leads to edema vision loss. The most common mutation in FECD an intronic CTG repeat expansion transcription factor 4 ( TCF4 ) its altered expression. Corneal wound healing occurs primarily through cell enlargement migration, CECs have been shown display increased migration speeds. In this study, we aim determine whether can promote cellular CECs. We generated stable CEC lines derived from patients overexpressed different isoforms investigated epithelial-to-mesenchymal (EMT) expression, analysis found full length TCF4-B isoform overexpression promotes EMT-independent manner. RNA-sequencing identified several pathways including negative regulation microtubules, with TUBB4A (tubulin beta 4A class IVa) as top upregulated gene. expression was ex vivo specimens, there cytoskeleton proteins, tubulin actin, compared normal healthy donor specimens. Corneal graft detachment is a major postoperative complication of Descemet's stripping automated endothelial keratoplasty (DSAEK). When corneal becomes detached, function generally fails, and repeat transplantation required. Purpose: To examine clinical results up to 10 years after Descemet Membrane Endothelial Keratoplasty (DMEK) and Triple-DMEK. Methods: Prospective study including 201 eyes, of which 54 eyes [38 DMEKs 16 Triple-DMEKs; Fuchs endothelial dystrophy (n = 45), pseudophakic bullous keratopathy 9)] reached the minimum follow-up 5 were followed for years. We evaluated best-corrected visual acuity (BCVA, logarithm angle resolution [logMAR]), cell density (ECD, cells/mm 2 ), minimal central corneal thickness (CCT, μm), retinal (μm), rebubbling, graft survival re-DMEK rate, chamber alterations, incidence glaucoma.
