Abstract BACKGROUND The number of people with dementia is increasing in Japan, and establishing evidence for preventing necessary. Intervention adherence is crucial to ensure cognitive benefits in trials designed prevent decline. During the coronavirus disease 2019 (COVID-19) pandemic, Japan-Multimodal Trial for Prevention of Dementia offered live online exercise sessions older adults with mild impairment (MCI). Background Dementia impacts individuals, families, and society, necessitating effective prevention strategies. Objective To evaluate the feasibility of a community-adapted multi-domain intervention for dementia among older adults in Obu City, Japan how uncertainties implementing definitive trials can be reduced. Methods A 12-month one-arm trial was conducted with 80 community-dwelling aged 65–86 years from two district regions. The included physical exercise, nutrition guidance, cognitive training, social participation, vascular risk management. primary outcome continuation rate, defined as proportion participants attending >60% classes initial assessment to 6 months. Secondary outcomes, such fidelity, acceptability, appropriateness, were assessed through qualitative quantitative evaluations. Additionally, health including function overall lifestyle, evaluated. Abstract Preventing dementia and Alzheimer’s disease (AD) is a global priority. Multimodal interventions targeting several risk factors mechanisms simultaneously are currently being tested worldwide under the World-Wide FINGERS (WW-FINGERS) network of clinical trials. Adherence to these crucial for their success, yet there significant heterogeneity in adherence reporting across studies, hindering understanding barriers facilitators. This article narrative review available evidence from multimodal prevention A literature search was conducted using medical databases (MEDLINE via PubMed SCOPUS) select relevant studies: nonpharmacological (i.e., combining three or more intervention domains), individuals without dementia, changes cognitive performance and/or incident mild impairment as primary outcomes.
