Abstract Aims Benefits of pharmacologic omega-3 fatty acid administration in cardiovascular prevention are controversial. Particularly, effects on coronary revascularization unclear; also debated specific benefits eicosapentaenoic (EPA). We investigated incident revascularizations, myocardial infarction (MI), stroke, heart failure (HF), unstable angina, and death, subjects randomized to receive EPA or + docosahexaenoic (EPA DHA) vs. control. Methods results Meta-analysis controlled trials (RCTs) was conducted after MEDLINE, Embase, Scopus, Web Science, Cochrane Library search. Preferred Reporting Items for Systematic Reviews guidelines were followed abstracting data assessing quality validity. Data pooled using a random model. Eighteen RCTs with 134 144 participants (primary secondary prevention) receiving DHA (n = 52 498), alone 14 640), control/placebo 67 006) included. Follow-up ranged from 4.5 months 7.4 years. Overall, compared controls, supplementation reduced the risk [0.90, 95% confidence interval (CI) 0.84–0.98; P 0.001; P-heterogeneity 0.0002; I2 68%], MI (0.89, CI 0.81–0.98; 0.02; 0.06; 41%), death (0.92, 0.85–0.99; 0.13; 33%). Background/Objectives: The triglyceride/high-density lipoprotein (TG/HDL) ratio serves as a simple marker for insulin resistance. We investigated whether the TG/HDL would be associated with incidence risk of heart failure (HF). Methods: study utilized data from National Health Insurance Service-Health Screening Cohort database South Korea 2002 to 2019. was time-dependent covariate or average value at least three times throughout follow-up period. outcome interest incident (HF) corresponding International Classification Disease, Tenth Revision code I50. Results: A total 293,968 individuals were included in this study. During median 9.6 years (interquartile range 9.2–10.13), 27,852 (9.47%) had cumulative HF. Considering multivariable Cox proportional hazard model repeated measures ratio, per unit increase significantly increased HF entire cohort (hazard (HR): 1.007, 95% confidence interval (CI): 1.002–1.011), diabetes mellitus (DM) (HR: 1.006. CI: 1.002–1.010), and non-DM 1.008, 1.003–1.013). Regarding quartiles, compared lowest quartiles (Q1), highest (Q4) positively accompanied by significant p trend 1.114, 1.075–1.155) fully adjusted analysis. Cardiovascular disease (CVD) that includes myocardial infarction and stroke, is the leading cause of mortality worldwide. Atherosclerosis, primary underlying CVD, can be controlled by pharmacological dietary interventions, including n-3 polyunsaturated fatty acid (PUFA) supplementation. PUFA supplementation, primarily consisting eicosapentaenoic (EPA) docosahexaenoic (DHA), has shown promise in reducing atherosclerosis modulating risk factors, triglyceride levels vascular inflammation. PUFAs act replacing pro-inflammatory types cell membranes plasma lipids, regulating transcription factor activity, inducing epigenetic changes. EPA DHA regulate cellular function through shared differential molecular mechanisms. Large clinical studies on have reported conflicting findings, causing confusion among public health professionals. In this review, we discuss important factors to these inconsistencies, context atherosclerosis, study design effects function. We propose steps improve basic experimental order supplement composition optimization. Finally, understanding poor response PUFAs, development biomarkers for predicting may help towards a more personalized treatment.

Objective. To compare insulin surrogate indices with the homeostasis model assessment of resistance (HOMA-IR) in Thai people type 2 diabetes (T2D). Methodology. A cross-sectional study 97 individuals T2D was done to determine association between HOMA-IR and seven for resistance. IR defined as ≥2.0. The included Waist Circumference (WC), Waist-to-Hip Ratio (WHR), Waist-to-Height (WHtR), Triglyceride-Glucose (TyG) index, estimated Glucose Disposal Rate (eGDR) calculated by WC, BMI, WHR. Results. total subjects (36.1% female, mean age 61.7 ± 12.0 years, BMI 26.4 3.7 kg/m2, A1C 6.9 1.2%) were studied. TyG index showed a positive HOMA-IR, while eGDR exhibited negative association. had strongest correlation (r = 0.49), various formulas weaker correlations 0.12-0.25). However, subgroup analysis coronary artery disease (CAD) that only eGDR-WC eGDR-BMI demonstrated significant triple vessel disease. Conclusion. useful simple marker identifying presence T2D.
