Embryonic genetic mechanisms are present in the brain and ready to be placed into action upon cellular injury, termed response injury wound-healing (RTIWH) mechanism. When injured, regional endothelial cells initially undergo activation dysfunction with initiation of hemostasis, inflammation (peripheral leukocytes, innate microglia, perivascular macrophage cells), proliferation (astrogliosis), remodeling, repair, resolution phases if injurious stimuli removed. In conditions wherein chronic, as occurs obesity, metabolic syndrome, type 2 diabetes mellitus, this process does not there is persistent RTIWH remodeling. Chronic obstructive pulmonary disease (COPD) is a progressive and debilitating condition characterized by airflow limitations systemic inflammation. The interaction between the metabolic inflammatory pathways plays key role in progression, with leptin insulin emerging as pivotal regulators. Leptin, an adipokine that regulates energy homeostasis, insulin, primary regulator of glucose metabolism, are both altered COPD patients. This narrative review provides in-depth examination roles pathogenesis, focusing on molecular mechanisms through which these regulators interact how their dysregulation contributes to spectrum extrapulmonary manifestations. These disturbances not only exacerbate symptoms but also increase risk comorbidities such syndrome, diabetes, cardiovascular disease, or muscle wasting. By exploring underlying COPD, this underscores significance metabolic-inflammatory axis, suggesting restoring balance modulation could offer novel therapeutic strategies for improving clinical outcomes.

Type 2 Diabetes (T2D) is a complex metabolic disorder that affects multiple organs, leading to severe complications in the pancreas, kidneys, liver, and heart. Prolonged hyperglycemia, along with oxidative stress chronic inflammation, plays crucial role accelerating tissue damage, significantly increasing risk of diabetic such as nephropathy, hepatopathy, cardiovascular disease. This review evaluates protective effects various antidiabetic treatments on organ tissues affected by T2D, based findings from experimental animal models. Metformin, first-line agent, has been widely recognized for its ability reduce inflammation stress, thereby mitigating diabetes-induced damage. Its extends beyond glucose regulation, offering benefits improved mitochondrial function reduced fibrosis tissues. In addition traditional therapies, new classes drugs, including sodium-glucose co-transporter-2 inhibitors glucagon-like peptide-1 (GLP-1) receptor agonists not only improve glycemic control but also exhibit nephroprotective cardioprotective properties reducing glomerular hyperfiltration, inflammation. Similarly, GLP-1 have associated hepatic steatosis enhanced function. Preclinical studies suggest tirzepatide, dual GLP-1/gastric inhibitory polypeptide agonist may offer superior compared conventional improving β-cell function, enhancing insulin sensitivity, fatty liver progression. Despite promising preclinical results, differences between models human physiology pose challenge. Further clinical research needed confirm these refine treatment strategies. Metabolic syndrome (MetS) is a major global public health concern due to its rising prevalence and association with increased risks of cardiovascular disease type 2 diabetes. Emerging evidence suggests that environmental chemical exposures may play significant role in the development MetS by disrupting metabolic pathways. This study used data from 2,960 participants Korean National Environmental Health Survey (KoNEHS) cycle 4 (2018-2020) examine associations between risk through machine learning (ML) approaches. Eight ML algorithms were applied, multilayer perceptron (MLP) random forest (RF) models identified as optimal predictors. The MLP achieved an AUC 0.79, RF highest F1 score 0.82. Both highlighted PFOA PFOS, alongside age BMI, key SHapley Additive exPlanations (SHAP) partial dependence plots (PDP) revealed both linear nonlinear exposure-response patterns, suggesting threshold effects for chemicals. These findings underscore importance incorporating into assessments. provided robust predictive performance novel insights interactions, advocating regulatory measures reduce harmful integrate factors prevention strategies.
