The association between air pollution (AP) and gestational diabetes mellitus (GDM), especially different pollutants GDM, remains controversial debatable. Hence, we conducted this systematic review meta-analysis to provide comprehensive evidence-based support for the AP GDM.The databases of Cochrane Library, Embase, PubMed, Web Science were searched from inception 1 April 2022, in combination with manual retrieval. Newcastle-Ottawa Scale (NOS) was used assess quality case-control studies cohort studies, while Joana Brigg's Institute (JBI) critical appraisal checklist assessment cross-sectional studies.We identified 35 epidemiological (including 33 study, study) covering 6,939,725 pregnant women, whom 865,460 GDM patients. NOS score all included higher than six, one rated as high according JBI assessment. Meta-analysis showed that fine particulate matter [PM2.5, odds ratio (OR) =1.06, 95% confidence interval (CI): 1.05-1.08, Z =7.76, P<0.001; PM10, OR CI: 1.01-1.11, =2.62, P=0.009; sulfur dioxide (SO2), =1.18, 1.10-1.26, = 4.69, nitric oxide (NO), =1.04, 1.03-1.06,Z =3.33, P=0.001; nitrogen oxides (NOX), =1.07, 1.04-1.11, =3.93, black carbon (BC), =1.08, 1.06-1.10, =7.58, P<0.001] associated GDM. Furthermore, no significant observed O3, CO, (NO2) exposure GDM.Exposure PM2.5, SO2, NO, NOX, BC significantly increases risk is a remediable environmental trigger can be prevented by human interventions, such lowering levels or limiting pollutants. government should strengthen supervision make information more transparent. As gestational diabetes mellitus (GDM) rises as a major public health concern, various factors have been identified potential contributors, with air pollution drawing increasing attention. The mechanisms by which pollutants lead to detrimental impacts are largely attributed oxidative stress.
