Diagnosis of pathology in the mediastinum has proven quite challenging, given wide variability both benign and malignant diseases that affect a diverse array structures. This complexity led to development many different non-invasive invasive diagnostic modalities. Historically, diagnosis relied on imaging modalities such as chest X-ray, computed tomography (CT), magnetic resonance imaging, positron emission topography. Once suspicious lesion was identified with one these techniques, gold standard for mediastinoscopy staging disease. More recently, minimally techniques CT-guided biopsy, endobronchial ultrasound transbronchial needle aspiration, endoscopic fine aspiration have revolutionized mediastinum. review provides comprehensive analysis all available diagnosing mediastinal disease an emphasis bronchoscopic techniques. Literature search performed via PubMed database. We included types articles study designs, including original research, meta-analyses, reviews, abstracts. Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is an established technique for lung cancer staging and the diagnosis of mediastinal diseases. Recently, paradigm EBUS guided sampling with conventional cytology has shifted over to histology specimens through use cryobiopsy. This case series explores novel technique, key enablers, potential advantages endoscopic ultrasound bronchoscope-guided transesophageal cryobiopsy (EUS-B-TEC). The findings this suggest that EUS-B-TEC a safe valuable addition bronchoscopic procedural armamentarium. Further studies are warranted validate EUS-B-TEC.
