Disturbances in sleep and circadian rhythms are common among residents of long-term care facilities. In this systematic review, we aim to identify evaluate the literature documenting outcomes associated with non-pharmacological interventions improve nighttime residents. The Preferred Reporting Items for Systematic Reviews guided searches five databases (MEDLINE, Embase, CINAHL, Scopus, Cochrane Library) articles reporting results experimental or quasi-experimental studies conducted settings (nursing homes, assisted-living facilities, group homes) which was subjectively objectively measured as a primary outcome. We categorized each intervention by its intended use how it administered. Of 54 included evaluating effects 25 different interventions, more than half employed randomized controlled trial design (n = 30); others used pre-post 11) without 13) comparison group. majority trials were at low risk most types bias, other met standard quality criteria. environmental 14), complementary health practices 12), social/physical stimulation 11), clinical 3), mind-body 3). Although there no clear pattern positive findings, three had promising results: increased daytime light exposure, melatonin, acupressure. Non-pharmacological have potential Further research is needed better standardize such provide implementation guidelines using cost-effective practices.

Dementia with Lewy bodies (DLB) is a complex disease that involves variety of cognitive, behavioral and neurological symptoms, including progressive memory loss, visual hallucinations, parkinsonism, cognitive fluctuations rapid eye movement sleep behavior disorder (RBD). These symptoms may appear in varying combinations levels severity each patient who seen the clinic, making diagnosis treatment challenge. DLB third most common all neurodegenerative diseases behind both Alzheimer's Parkinson's (PD). The median age onset for (76.3 years) younger than PD dementia (81.4 years). New pathological studies have shown patients variable amounts changes their brains, explaining wide variability this disease's clinical presentation course. This review discusses three cholinesterase inhibitors been to be effective managing DLB: rivastigmine, galantamine donepezil. Memantine able improve global impression change those mild moderate DLB. Levodopa can treat parkinsonism some patients, but dose often limited due fact it cause agitation or worsening hallucinations. A recent phase 2 trial showed benefit zonisamide when added as an adjunct levodopa treating parkinsonism. While atypical antipsychotic drugs not always helpful monotherapy associated DLB, low doses valproic acid like quetiapine. Pimavanserin prove useful psychosis other are used there small increased risk mortality. RBD, which core feature managed either melatonin clonazepam. Two agents targeting alpha-synuclein (NPT200-11 ambroxol) currently hold promise disease-modifying therapies they yet tested trials. An agent (E2027) offers hope neuroprotection by increasing central cyclic guanosine monophosphate (cGMP) being examined trials patients.

Sleep and circadian rhythms are among the most powerful but least understood contributors to cognitive performance brain health. Here we capitalize on resetting effect of blue-wavelength light phase shift sleep patterns adult patients (aged 18-48 years) recovering from mild traumatic injury (mTBI), with aim facilitating recovery structure, connectivity, performance. During a randomized, double-blind, placebo-controlled trial 32 adults recent mTBI, compared 6-weeks daily 30-min pulses blue (peak λ = 469 nm) each morning versus amber placebo 578 neurocognitive neuroimaging outcomes, including gray matter volume (GMV), resting-state functional directed connectivity using Granger causality, white integrity diffusion tensor imaging (DTI). Relative placebo, led phase-advanced timing, reduced daytime sleepiness, improved executive functioning, was associated increased posterior thalamus (i.e., pulvinar), greater thalamo-cortical axonal these pathways. These findings provide insight into contributions systems in repair lay groundwork for interventions targeting retinohypothalamic system facilitate recovery.

Light can impact human health via the visual and non-visual systems originating in retina of eye or as optical radiation falling on skin. This paper provides a summary what is known about these impacts. Topics covered include aspects lighting likely to cause eyestrain headaches; increase risk falls; induce circadian disruption; enhance sleep; mitigate Alzheimer’s disease depression; produce tissue damage; alleviate diseases through phototherapy inactivate viruses germicidal irradiation. It concluded that undoubtedly influenced by lighting, but there are four conditions should be attached such simple assertion. First, either positive negative. Second, affected many factors other than lighting. Third, severity effects light exposure vary widely from short-lived trivial long-term fatal. Fourth, same have very different impacts for individuals depending their age medical status. Taken together, imply care required when judging veracity relevance broad assertions benefits health.
