Purpose: Chronic obstructive pulmonary disease (COPD) is a prevalent respiratory condition characterized by chronic airway inflammation, where macrophages from the innate immune system may exert pivotal influence. The respiratory mucus, a viscoelastic gel, effectuates primary line of the airway defense when operated by mucociliary clearance. In chronic diseases (CRDs), such as asthma, obstructive pulmonary disease (COPD), and cystic fibrosis (CF), mucus is overproduced its solid content augments, changing structure properties determining derangement essential mechanisms against opportunistic microbial (virus bacteria) pathogens. This ensues in damaging airways, leading to vicious cycle obstruction infection responsible for harsh clinical evolution these CRDs. Here, we review features normal pathological (i.e., sputum CF, COPD, asthma), i.e., mucin content, (mesh size), micro/macro-rheology, pH, osmotic pressure, ending with awareness that biomarkers (mucins, inflammatory proteins peptides, metabolites) might serve indicate acute exacerbation response therapies. There are some indications old novel treatments may change structure, properties, biomarker sputum; however, wealth work still needed embrace measures correlates severity association (or even substitutes of) functional tests.

Although Chronic Obstructive Pulmonary Disease (COPD) is highly prevalent, it often underdiagnosed. One of the main characteristics this heterogeneous disease presence periods acute clinical impairment (exacerbations). Obtaining blood biomarkers for either COPD as a chronic entity or its exacerbations (AECOPD) will be particularly useful management patients. However, most earlier studies have been characterized by potential biases derived from pre-existing hypotheses in one more their analysis steps: some only targeted molecules already suggested knowledge, and others had initially carried out blind search but later compared detected among well-predefined groups. We hypothesized that clinically cluster on results non-hypothesis-driven wide proteomic would determine an unbiased grouping patients, potentially reflecting endotypes and/or characteristics. To check hypothesis, we included plasma samples 24 stable 10 additional patients with AECOPD, healthy controls. The were analyzed through label-free liquid chromatography/tandem mass spectrometry. Subsequently, Scikit-learn machine learning module K-means used clustering individuals based solely profiles. obtained clusters confronted groups at end entire procedure. our unable to differentiate individuals, they segregated those AECOPD conditions (sensitivity 80%, specificity 79%, global accuracy, 79.4%). Moreover, proteins involved process identify associated five biological processes: inflammation, humoral immune response, coagulation, modulation lipid metabolism, complement system pathways. Even though present merit external validation, suggest blinded approach may segregate stability both setting trials, favoring personalized medicine research.
