Background Parkinson’s disease (PD) is the second most common neurodegenerative disorder, and it has an unclear pathogenesis lacks validated, specific biomarker-based diagnostic approaches, particularly in PD patients with rapid eye movement (REM) sleep behavior disorder (PD-RBD). Methods Using untargeted liquid chromatography-mass spectrometry (LC–MS) metabolomics, serum profiles of 41 drug-naïve [including 19 PD-RBD 22 without RBD (PD-nRBD) patients] 20 healthy controls (HCs) were analyzed. Results Comparative analyses revealed 144 dysregulated metabolites versus HCs, 7 metabolites—sodium deoxycholate, S-adenosylmethionine, L-tyrosine, 3-methyl-L-tyrosine, 4,5-dihydroorotic acid, (6Z)-octadecenoic allantoin—demonstrating high classification accuracy [area under curve (AUC) > 0.93]. Compared PD-nPBD patients, exhibited distinct metabolic profiles, characterized by 21 differentially expressed metabolites, including suberic methyl (indol-3-yl) acetate (AUC 0.86). Notably, 3-methyl-L-tyrosine displayed dual dynamics, reflecting dopaminergic depletion compensatory adaptations PD-RBD. Pathway enrichment analysis implicated central carbon metabolism (CCM) disruption peroxisome proliferator-activated receptor (PPAR) signaling pathway inactivation Conclusion These findings reveal potential serum-based biomarkers for PD-RBD, highlight CCM PPAR pathways as therapeutic targets, underscore role dysregulation pathophysiology.

Abstract Rapid eye movement behavior disorder (RBD) is a parasomnia characterized by the loss of skeletal muscle atonia during rapid (REM) sleep phase. On other hand, idiopathic RDB (iRBD) considered prelude various α‐synucleinopathies, including Parkinson's disease (PD), dementia with Lewy bodies and multiple system atrophy. Consequently, over 40% patients eventually develop PD. Recent neuroimaging studies utilizing structural magnetic resonance imaging (s‐MRI), diffusion‐weighted (DWI), functional (fMRI) graph theoretical analysis have demonstrated that iRBD extensive brain abnormalities. Thus, it crucial to identify new biomarkers aid in determining underlying physiopathology group. Goal: REM Sleep Behavior Disorder (RBD) is a parasomnia that associated to high risk of developing ⍺-synucleinopathies, as Parkinson's disease (PD) or dementia with Lewy bodies, over time. This study aims at investigating the presence autonomic dysfunctions in RBD subjects, and without PD, by assessing their sleep structure autonomous nervous system activity along different stages. Methods: To this aim, an innovative framework combining transition model, Markov chains, instantaneous assessment state dynamics statistical modeling heart rate variability (HRV) through point-process approach, was introduced. Results: In general, groups showed lower HRV than controls across all stages, well higher probabilities transitioning towards lighter Subjects also affected PD present even HRV, but better continuity. Conclusions: patients suffer from fragmentation overall dysfunction, mainly due activation Coexistence seems improve quality, possibly sleep-related relief symptoms.
