Background Substance use disorders (SUDs) result in individual and societal burden. However, most individuals with SUD receive no treatment. Implementing interventions primary care could address this population's treatment needs. In the USA, reSET ® reSET-O were first prescription digital therapeutics (PDTs) for SUDs opioid disorder (OUD), respectively. The Digital Treatments Use Disorder (DIGITS) study tested effectiveness of practice facilitation health coaching strategies to support implementation into care. A formative evaluation was conducted monitor implementation, inform adaptations, learn what promotes PDT sustainment. Method Dynamic Sustainability Framework Reporting Adaptations Modifications Evidence-based Implementation Strategies guided evaluation. Using rapid qualitative methods, we collected analyzed observational fieldnotes, key informant interviews, document sources (e.g., meeting minutes) synthesis dissemination clinical partners team via reports. We reports generate results. Results Twenty-four clinics participated. Evaluation data included 98 16 253 sources. produced nine encountered barriers facilitators each DSF domain (ecological system, setting, intervention). ecological vendor enabled study, but COVID-19 pandemic, laws, regulations, contracting delayed implementation. staff shortages low clinic capacity challenges, while electronic record capabilities both facilitators. At intervention level, non-routine workflows, clinician burden, patient engagement despite clinicians’ efforts. Conclusions are promising OUD treatments, integration requires conducive laws organizational capacity, engagement. Formative identified important lessons future
