Abstract Objectives Many countries have continued to experience a higher-than-expected number of deaths following the peaks in mortality observed first year Covid-19 pandemic. This scoping review aims identify different explanations proposed for sustained beyond pandemic year. Study design Scoping Methods A systematic search databases and grey literature sources was completed English-language records proposing or investigating hypotheses from April 2021 onwards (sub)populations high-income countries. Papers focused on survival diagnosis intervention were excluded. Results summarised narratively, existing research prioritisation frameworks adapted applied as highest priority further research. Seventy eligible papers identified. Most opinion pieces simply presented trends; few included investigation suggested hypotheses. Numerous proposed, with relating direct mortality, sequalae infection, health service impacts pandemic, wider socioeconomic factors identified highest-priority Conclusions The causes are likely be multiple potentially interactive. will help shape into current trends, critical understanding this topic essential achieving evidence-informed policy.
