Vitamin D deficiency is a significant public health concern that affects bone and muscle function in children, especially developing countries. The COVID-19 pandemic has intensified this issue because lockdowns have reduced sunlight exposure. We report rare case of 13-year-old Indian boy who developed severe proximal myopathy induced by vitamin during the pandemic. patient presented with generalized body aches, progressive lower limb weakness, difficulty walking, waddling gait, positive Gower’s sign. Laboratory tests revealed hypovitaminosis (25[OH]D level, 3.8 ng/ml), hypocalcemia, hypophosphatemia, elevated parathyroid hormone, alkaline phosphatase levels. Electromyography nerve conduction study results were normal. was diagnosed deficiency-induced osteomalacia, likely due to exposure, inadequate dietary intake, obesity. treatment involved high-dose supplementation, oral calcium, lifestyle modifications, structured physiotherapy program focusing on resistance training functional mobility exercises. Despite biochemical normalization after 2 months, symptomatic improvement achieved only intensifying physiotherapy. By 7 had fully recovered strength, normal maintained follow-up laboratory values. This emphasizes importance considering children weakness periods limited highlights need for multidisciplinary approach effective management full recovery.
