An electronic search for publications on the features of course COVID-19 in patients with most common chronic noncommunicable diseases (arterial hypertension, diabetes mellitus and obesity) was carried out using Scopus, Web Science, MedLine, The Cochrane Library, EMBASE, Global Health, CyberLeninka databases, RSCI, as well journals peer-reviewed by Higher Attestation Commission. review analyzed effect comorbidity prognosis a new coronavirus infection (complication rate, severity course, mortality). It has been shown that previous are significant risk factor adverse outcomes COVID-19, if arterial hypertension is not confirmed all authors, then type 2 mellitus, any degree obesity, important prognostic signs an unfavorable infections. Thus, context pandemic, suffering from need more careful monitoring preventive measures aimed only at preventing coronavirus, but also slowing progression these pathologies their complications.

Through dynamic follow-up over the past year and a half, it has been found that patients with COVID-19 on background of hypertension suffer more severely from this disease often require medical care in intensive unit. In addition, laboratory studies showed significant increase levels D-dimer, LDH, C-reactive protein such patients. Patients coronavirus infection accompanied by had severe course pneumonia respiratory failure, development an excessive inflammatory reaction form cytokine storm was observed frequently. At same time, doctors believe is not directly related to high blood pressure, but fact suffering arterial have concomitant diseases as obesity diabetes, most cases belong older age group. Despite itself factor contributing complicated infection, pathology careful monitoring personnel cautious selection drug therapy.

Objective: to clarify the clinical and diagnostic features of arterial hypertension in patients with mild moderate forms COVID-19, taking into account elapsed time. Material methods. We examined 116 aged 44-60 years who were admitted hospital uncontrolled stage 2 hypertension, 96 them had COVID-19. The divided two groups: first group up 12 weeks after suffering COVID-19 second weeks. examination included echocardiography ambulatory blood pressure (BP) monitoring. Results. In post-COVID-19 patients, ratio daytime nighttime systolic BP was statistically significantly lower than disease-free (p <0,001, p1 =0,015). There a decrease daily index 1 (11.6±0.8% 12.2±1.2%, respectively, p=0,001). Among non-dippers detected, more often (9 people (17.6%) versus (4.4%) (x2=4.18, p=0.042). Conclusion. Arterial underwent characterized by an insufficient at night, presence positive dynamics from onset infectious process.

Comorbidity is an important problem of modern medicine, the study which a priority for public health. According to literature, prevalence comorbidity reaches third population in some countries, while there tendency increase number such patients all age groups. In context COVID-19 pandemic, presence significant risk factor affecting course and prognosis new coronavirus infection; at same time, it comorbid who belong most vulnerable group. The review presents data indicating impact pathologies on complications mortality; common combinations diseases with infection were analyzed. There groups, dictates need develop integrated interdisciplinary approach patients. vast majority clinical guidelines treatment comorbidities are mainly focused individual diseases, limits use these algorithms Due high contracting viral infection, as well frequent development mortality patients, preventive measures should be simultaneously two tasks: aimed preventing compensating pathology. case SARS-CoV-2 careful monitoring often hospital setting, necessary order prevent adverse outcomes.

Aim. To study the prevalence of comorbidity affecting choice antihypertensive treatment, as well efficacy and safety application a single pill combination lysinopril, amlodipine rosuvastatin (SPC LAR) in hypertensive patients. Material methods. Observational epidemiological with participation 626 doctors 13037 patients high cardiovascular risk dyslipidemia treated SPC LAR were held 24 cities Russia from November 2020 to March 2021. Prevalence types comorbidity, changes blood pressure (BP), total cholesterol (TC) glucose levels adverse events studied during observation. Patients had 3 visits – initially, after 4 12 weeks. The obtained data was recorded by through web portal. Results. Concominant diseases found all (in 38.7% cases 2 5 diseases). most commonly coronary artery disease (43.1%), metabolic syndrome (36.6%), diabetes (25.2%), chronic heart failure (24.9%), cerebrovascular (12.3%) obstructive pulmonary (0.5%). minimal doses components 41.1% patients) or reinforced hypocholesterolemic effects, based on experience patient prescribed starting treatment doctors. Reducing level systolic diastolic BP, Tc, respectively, 20.9%, 16.9% 29.7% study. Target BP achieved 97.6% patients, 99.6%, target TC ≤5 mmol/l, ≤4 mmol/l ≤3 achieved, 94.5%, 68.6% 23.5% No significant dynamics glycemia not found. tolerated Side effects 0.53% (more often there dry cough, feet edema headache). Commitment 92.7%. Conclusion. modern hypertension phenotype has atherosclerosis-associated diseases, which justifies need combine hypolipidemic therapy. Triple LAR, effectively controlled improved violation lipid metabolism this observational low frequency side good tolerance accompanied adherence treatment.

Objective : improving the effectiveness of hypertension control in patients after COVID-19 with manifestations depression. Materials and methods: study included 48 who had suffered a coronavirus infection. The criterion for inclusion was effective blood pressure when using two-component antihypertensive therapy before development disease its absence same at time study. To identify symptoms depression, conducted Beck Depression questionnaire. Group A consisted arterial depression group B-patients without In both groups, combination renin-angiotensin-aldosterone system blocker, diuretic calcium channel blocker were used. observation carried out 4 weeks, dynamics daily parameters, evaluated. Results presence detected 39.5% patients. At study, office exceeded target level all patients, there an insufficient decrease systolic diastolic during day night, increase most indicators variability. use three-component made it possible to achieve 93.1% only 21.0% reached level. analysis showed All statistically significantly higher than B. Design methods A retrospective analysis case histories 940 was performed. Statistical carried out by the method contingency tables using Pearson's Chi-square test Bonferroni correction, multiple logistic regression ROC-analysis were also applied. Results The group HTN showed a number significant compared without history. study characterized higher rates females, people over 65, COVID-19. They more likely have diabetes mellitus, coronary heart disease, chronic kidney disease. On admission, patients, leukocytosis elevated blood glucose levels common, pneumonia, shortness breath low saturation (p < 0,05). Multivariate that presence disease (odds ratio (OR) = 3,492, 95 % confidential interval (CI): 2,016-6,048), (OR 1,376, CI: 1,540-7,703), thrombocytopenia 1,779, 1,031-3,071) SpO 2 3,265, 1,996-5,341) hypertensive combination identified increases power their association (AUC 0,766). The developed standardized questionnaire CHRONOS19 study used. primary endpoint (adverse outcome) was a composite point that included mortality any cause during observation period, development acute respiratory distress syndrome, hospitalization intensive care unit, need mechanical ventilation. A features showed increase number neoplastic blood diseases, especially chronic lymphoproliferative myeloid leukemia. Significant predictors unfavorable include refractory/recurrent variant tumor, glucocorticoid therapy as protocol treatment underlying disease, stage 3-4 lung damage according computerised tomography scans at onset COVID-19, presence diabetes mellitus. Predictors have been identified. Hematological context pandemic require coordinated interdisciplinary approach involving hematologists therapists, careful monitoring clinical laboratory parameters reduce events.
