Background The epidemiology of childhood SARS-CoV-2 infection and COVID-19-related illness remains little studied in high-transmission tropical settings, partly due to the less severe clinical manifestations typically developed by children limited availability diagnostic tests. To address this knowledge gap, we investigate prevalence predictors (either symptomatic or not) disease 5 years-old Amazonian children. Methodology/Principal findings We retrospectively estimated attack rates proportion infections leading among 660 participants a population-based birth cohort study Juruá Valley, Brazil. Children were physically examined, tested for IgG IgM antibodies, had comprehensive health questionnaire administered during follow-up visit at age years carried out January June-July 2021. found serological evidence past 297 (45.0%; 95% confidence interval [CI], 41.2–48.9%) participants, but only 15 (5.1%; CI, 2.9–8.2%) seropositive prior medical diagnosis COVID-19 reported their mothers guardians. period clinically apparent COVID-19, defined as presence specific antibodies plus one more symptoms suggestive (cough, shortness breath, loss taste smell) guardians since pandemic onset, was 7.3% (95% 5.4–9.5%). Importantly, from poorest households those with educated significantly likely be seropositive, after controlling potential confounders mixed-effects multiple Poisson regression analysis. Likewise, 1.8-fold (95%, CI 1.2–2.6-fold) higher exposed food insecurity 3.0-fold 2.8–3.5-fold) born non-White mothers.
