Introduction: Although delusions in Parkinson's disease (PD) are rare, when they occur frequently take the form of "Othello syndrome": irrational belief that a spouse or partner is being unfaithful. Hitherto dismissed as either by-product dopamine therapy cognitive impairment, there still no convincing theoretical accounts to explain why only some patients fall prey this delusion, it persists despite clear disconfirmatory evidence. Methods: We discuss limitations existing explanations namely hyperdopaminergia-induced anomalous perceptual experiences and before describing how Bayesian predictive processing can provide more comprehensive explanation by foregrounding importance prior experience its impact upon computation probability. illustrate new conceptualisation with three case vignettes. Results: suggest those romantic betrayal, hyperdominergic-induced aberrant prediction errors enable accrue greater prominence, which then maintained through Bayes-optimal inferencing confirm distortions, eliciting shaping dangerous delusion. Conclusions: propose first mechanistic account Othello syndrome PD implications for clinical interventions.
