Knee osteoarthritis (OA) is a debilitating condition with limited long-term treatment options. The therapeutic potential of mesenchymal stem cells (MSCs), particularly those derived from bone marrow aspirate concentrate, has garnered attention for cartilage repair in OA. Background/Objectives: Osteoarthritis (OA) is a prevalent and debilitating joint disorder characterized by progressive cartilage degradation inflammation, for which traditional treatments offer only symptomatic relief without halting disease progression. Exosomes, cell-free vesicles derived from mesenchymal stem cells, have emerged as promising alternative therapy owing to their regenerative anti-inflammatory properties. Methods: This review synthesizes findings recent studies (2017–2023) on the therapeutic potential of exosomes in OA treatment, highlighting ability modulate microenvironment, reduce promote repair delivering bioactive molecules such cytokines, growth factors, regulatory ribonucleic acids. Results: We explore integration with biomaterials, hydrogels scaffolds, enhance delivery efficacy, we address critical challenges associated clinical application, including standardization isolation characterization methods, scalability production, mechanistic understanding, long-term safety.
